One strand of the No.6 traction cable broke and unwound causing the strand to bunch up on the drive motor sheave wheels. This caused the rope gripper brake and the car safety wedge brakes to engage. There were 4 individuals on the elevator when this occurred. No injuries were incurred.

All incidents in 2023
The employee was changing the bearings in the #24 tow motor. The machine was blocked, and a crib block placed under the hub and a bottle jack placed under the trunnion. After changing the top bearing, the employee raised the trunnion with the bottle jack to remove the crib. The trunnion slipped off the jack and caught EE's right forearm between the hub and the crib block.
As the EE was loading the roof bolter they stepped off the back of the machine to get more supplies and sprained an ankle.
The employee was carrying two full buckets of miner bits and tripped on a rock. They fell into a rib and felt pain in their back, legs, and pelvic region.
While loading a top roller into a scoop insert, EE stated felt a pop in lower back.
Two employees were hanging bottom roller on structure during a belt move. The roller was being lifted simultaneously by each employee with the bottom belt laying loose over top. The injured employee felt a pop in shoulder. Note: The employee worked until 6/12 when employee was then required to have surgery to repair damage sustained during this action.
Employee was installing the fork back onto the forklift. While trying to get it into place EE's left-middle finger was caught between the back-clip of the fork and the cleat of the fork-frame. EE received 2 stitches in the left middle finger.
Employee was in the process of positioning the drill station to drill a corner bolt on the outside of the entry when a rock/kettle bottom fell from the roof, sliding down the roof screen and struck EE in EE's back. The strike to EE's back pushed EE forward causing EE to hit mouth against the canopy.
While bolting, the employee was placing a piece of mesh on top of the ATRS and adjusting it with EE's hands. A rock fell and caught the inby corner of the mesh and caused the mesh to spin around and strike EE in the mouth area and knock out 2 teeth.
