Accident type, without injuries

An unplanned roof fall occurred in the secondary escapeway for the No.3 section. Located near survey station 22958, and was approximately 18'x 18' in area and estimated to be 10-12' in height. Agencies were notified promptly and employees withdrew from affected area.

Struck by falling object

Employee was operating a roof bolter and as employee pressurized the drill steel against the mine roof to begin drilling a hole a piece of rock fell and hit the employee on the left hand resulting in a laceration and fracture to the left ring finger and an amputation to the left middle finger at the first joint.

Caught in, under or between a moving and a stationary object

Employee was attempting to load a 20"" X 6"" I Beam. While doing so the beam rolled and caught employee's left index finger between it and another beam resulting in the amputation of the finger just behind the finger nail.

Employee was adjusting forks on forklift and pinched left ring finger between the forks and the slide bar.

Employee was handling the forks of a forklift and smashed finger between the fork and the frame

Employee was loading a stab jack for a roof bolter into a Wallace diesel mantrip with the help of another employee. The stab jack slipped out the employee's hand and caught left index finger between the stab jack and the side of the mantrip. This resulted in a fracture to the tip of the left index finger.

Employee was mining the 2nd cut out of 3 lt. crosscut when a piece of rock bounced out of the shuttle car EE was loading and struck EE in the mouth.
