An unplanned roof fall was encountered when traveling toward the number one section. It was located at survey station 5043. The fall was approximately twenty- five foot long, sixteen to eighteen foot wide, and five foot high.

The hoist man informed the evening shift mine foreman that he was unable to set the brakes up on the brake car. He thought it may be a sensor. He then called both the state and federal agencies to inform them of the issue. After maintenance was called in the issue was corrected and both agencies was called back and informed of the status.
An unplanned roof fall occurred on the number 3 section. It was in the number two entry approximately inby 80' survey station 14233.
At 7:50am an unplanned roof fall was encounterd in the secondary escapeway for the number one section. It began approxiamately 50' inby survey station 5261 and extended 5' into the intersection of survey station 5278. The fall was 25-30' long, 16' wide and appeared to be 5-6' in height. All aproaches were dangered and breakered off. MSHA was notified at 8:00am.
An unplanned roof fall was encountered in the First Right Panel off of the South West Mains Section of the mine. The fall occurred near survey station 1675 and extended outby a distance approximately fifty foot. The size varied from five to six foot in thickness. All approached were previously dangered and breakered off. The fall has been marked on the mine map as required.
The employee was hit in arm by a piece of rock while drilling the hole for a roof bolt.
Employee was preparing to unload a plate press panel from a bed of a truck when a large gust of wind blew three panels over, striking him in the low back and knocking him down.
Employee was unloading 600 pounds lift plates when wind blew three plates over hitting employee knocking him down.
Employee states while drilling the foot jack slipped off of a rock and caused the canopy to hit his head.
Employee was driving a tow motor to 1 section. Near spad 5784 he encountered 2 rope hangers tied together hanging from the roof. His neck became caught by these hangers. He was treated and released. Returned to the doctor on 3-15 and was released. On 3-17 he went to his family doctor which referred him to a specialist who put him off of work. His first missed day was 3-18.
The employee was removing a rub rail and it slipped off, and fell on his left foot. This report is being filed to terminate citation # 8149176.
Employee was bolting the face of the #2 entry on the #3 section and when he put his drill steel against the roof a piece of rock came loose and hit the ATRS which deflected the rock toward him striking him on his right arm above the elbow resulting in a laceration.
An employee of our operation was diagnosed with pneumoconiosis and has exercised his part 90 right to transfer to a less dusty area of the mine where the concentration of dust is less than 1 milligram per cubic meter.
