All MSHA-reportable accidents at Jim Walter Resources Inc operations in 2008. Fatalities appear first.
IGNITION: The Miner was cutting a large pot that had fallen out of the mine roof in the middle of the No. 2 Entry Face, when the ignition occurred. The flame was on the operator side of the Miner Head. The flame was put out with one fire extinguisher.
IGNITION- As the electrician was burning weld from the H/G drum, a spark from the torch fell into a crack in the footwall and set a bleeder on fire. The flame lasted 6 seconds and was extinguished with water. No one was hurt nor was their any property damage incurred.
IGNITION: The Continuous Miner was cutting coal in the No. 1 Entry on No. 8 Section and while cutting close to the roof, a frictional methane ignition occurred. The ignition lasted approximately 2 minutes and was extinguished with 2 fire extinguishers and a water hose.
IGNITION: While cutting and cleaning up coal in the No. 3 Face on the No. 8 Section, a frictional gas ignition occurred. The flame was put out with a fire extinguisher and lasted approximately 20 seconds.
Ignited methane while welding hole in shroud of deep well pump.
IGNITION. The Continuous Miner was cutting the bottom rock in the No. 3 Entry on the No. 4 Section on the curtain side of the Face when a frictional ignition occurred.
IGNITION. The Continuous Miner was cutting coal on the No. 8 Section in the cross cut between the No.2 & No.3 entry on the curtain side when they saw a flame on the mine floor. They put the flame out with a water hose and one fire ext.
IGNITION-- The Longwall Shearer Machine was trying to cut a large rock at No. 101 Shield when the H/G Shearer Opr. saw a flash around the drum of the Shearer. When the machine and drum was cut off the flash went out. It lasted approximately 30 seconds.
IGNITION--The Continuous Miner was cutting in the No 4 Entry. While cutting in the middle of the face, an ignition occurred. The ignition lasted less than a minute according to the Miner Opr. Used wash down hose and sprays to put fire out.
Cutting coal in the No. 1 Entry on No. 8 Section and while cutting coal on left sidt of entry, an ignition occurred. Ventilation was disrupted by the Ramcar just prior to the ignition. Fire was put out with a fire extinguisher. The ignition lasted approximately 30 seconds.
The continuous miner was cutting coal in the No. 1 Entry on the No. 8 Section and a frictional ignition occurred. It lasted approximately 30 to 60 seconds and was put out with a water hose.
The Miner was cutting coal in the No. 4 Face on the No. 8 Section and a frictional ignition occurred that lasted approximately 1 minute and was put out with a fire extinguisher.
Longwall shearer was mining at No. 17 shield when an ignition occurred under the panline at No. 8 shield.
The continuous Miner was cutting coal and rock in the No. 1 Entry on the No. 8 Section and while loading the last Ramcar, a frictional ignition occurred on the right side of the entry next to the face
Roof fell approximately 3 feet above the anchorage of the 6 foot Resin Bolts in the No. 2 Entry and across the intersection into the No. 1 Entry on the No. 10 Section.
Using torches in the shaft. Hot embers fell down the shaft, caught bleeder on fire. Took about 5 minutes to extinquish. Called federal inspectors. Cleared to return to work at 4:00 p.m.
A methane ignition occurred in the face area while grading bottom. No injuries were incurred.
While shortening service cage tailropes, east side floor of elevator was damaged when chaired. NO INJURIES.
Inby brow area of recently shot overcast fell above anchorage, impeding travel in primary escapeway on #6 Sect.,MMU 006, Spad 24876. NO INJURIES.
Cutting coal and rock in the No. 2 entry on the No.7 Section when a frictional methane ignition occurred.
Zero methane check. Employees were 150 feet above fire. Hot embers fell down shaft, caught bleeder on fire. Took about 3 minutes to extinquish fire. Called in. On D order until inspector came out. Resumed work approximately midnight.
The Joy Miner was cutting bottom in the No 3 Entry on No. 6 Section that was driving the South Entries when an ignition occurred in the bottom of the Face. It took approximately one minute to put the fire out. This was a frictional ignition.

While cutting with a torch on the feeder in the Raw Coal Tunnel at the Preparation Plant, an ignition occurred. A flame ignited and then immediately went out.
The Joy Miner shear down to cut the bottom rock and a frictional ignition occurred in the No. 2 Face on the No. 6 Section.
Chemical foam was sprayed on cavity and heated up during expansion. The heat created a small fire.
Struck on right hand by a rock that was thrown off HG Drum. Partial Amputation of right middle finger.
Employee put his hand in harms way and the tip of his right ring finger was cut off.
EMPLOYEE CHECKING ALTERNATOR, REACHED TO REV ENGINE AND HAND WAS PULLED INTO ALTERNATOR, AMPUTATION LEFT INDEX FINGER.
Employee was operating ram car when he ran over a rock injuring lower back. Did not start losing time until 7/17/09.
DID NOT START LOSING TIME UNTIL 04/30/2009. SURGERY ON 4/30/09. Jacking dust tank, slipped and felt pain in right knee.
While setting 4 x 4 timbers, one fell, cutting his upper and lower lip and breaking a tooth.
Employee was pushing a wheelbarrow along bunker and the wheel hit a rib and then fell over on the employee's foot.
Building an overcase wall. Wall fell on employee's left lower leg and foot.
While climbing off old N-9 drive, employee slipped and fell on I beam. Broke left lower arm, surgery required.
Employee was pulling slack from rope for monorail and tripped on a rock and fell and twisted his left knee. EMPLOYEE DID NOT START LOSING TIME UNTIL 2/5/09 DUE TO SURGERY
Removing check valve from line, coupling blew off and hit him above right eye, knocked him down and hurt his left little finger.
Employee was pulling the rockdust hose and tripped and fell hurting left shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 2/5/2009 DUE TO SURGERY.
Employee was climbing over side of stageloader and lost footing and slipped and caught himself straining left shoulder and upper arm. EMPLOYEE DID NOT START LOSING TIME UNTIL 5/27/09 DUE TO SURGERY.
Employee was walking and stepped on something that made his right ankle twist.
THE EMPLOYEE WAS MAKING PRE SHIFT SLIPPED AND FELL HURTING HIS BACK.
Employee was lifting ram jack and the jack came off the chain and hit employee's left foot. **Employee started losing time again on 2/23/09 due to surgery**
While degreasing the roofbolter, the water hose blew apart and struck the employee on the left knee causing pain and swelling.
EMPLOYEE PULLED ON THE STAND PIPE AND STRAINED HIS RIGHT ELBOW.
Employee was running the Ramcar and hit a hole causing him to strike his head against the canopy.
Employee was on the motor and it derailed causing employee to be struck in the head by the water manifold which was hanging from the roof.
Building a brattice. Brattice fell hitting his head, right hand and left leg.
Moving a beam and a clip fell and landed on his hand
Employee was on motor and the car stop stuck and the bar used to free the car stop slipped and hit employee in the face
Employee walked over to employees hammering roof bolt pin to instruct them to wear their safety glasses, when a piece of metal flew out and stuck employee in the chest.
DID NOT START LOSING TIME UNTIL 10/1/08. Stepped off landing, stepped on loose coal and slipped and fell, hurting his left shoulder. Surgery on 10/1/08.
Employee was walking thru the Belt Entry and slipped and fell in mud hole and sprained his left knee. EMPLOYEE DID NOT START LOSING TIME UNTIL 9/17/2008
Employee was walking and ran into a low shield canopy hurting his neck. EMPLOYEE DID NOT START LOSING TIME UNTIL 10/23/2008 DUE TO SURGERY.
Employee was checking a pump that was being lifted out of water and when he reached out to lift one end up to inspect he felt pain in lower back. (EE HAS BEEN WORKING SINCE DATE OF ACCIDENT THAT OCCURRED AT JWR #4 MINE ID 0101247 AND HAS BEEN TREATED STEADLY FOR THE INJURY. EE IS CURRENTLY AT JWR #7 MINE ID 0101401 AND DID NOT START LOSING TIME UNTIL 12/17/15 DUE TO SURGERY).
Employee was walking on rough surface in the dark, when he turned he twisted his right knee. However the employee did not start losing time until his Saturday Night Owl shift on 9/12/2008. The employee is scheduled for surgery later this week.
Employee was connecting relay bar to pan, and the chain on the back of the ram made relay bar jump up and it crushed left finger

Employee was on the side of the scoop dumping; cable hooks pulled loose and hit employee in the back forcing him into the duster on the scoop.
As employee was helping another employee carry a timber, the timber fell and struck him on the left foot, resulting in a deep bruise and swelling making him unable to wear his work boot.
Employee got something in his right eye while pulling the curtain
While backing the roofbolter out of the entry, a cable bolt (8' long) was caught in rib on left side of entry and it flipped around and hit the employee in the face.
DID NOT START LOSING TIME UNTIL 8/25/08. Started to slip, raised up, hit head on feeder. Surgery required on neck.
He was operating shuttlecar, loading behind continuous miner. Miner hit tail of sc. He hit his head in top of canopy. Strained back.
Employee had come-a-long hooked to a bracket on each side of tilt cylinder, holding it up to change out pins. Employee was using his finger as a line-up tool. The bracket broke and caught his fingers between the cylinder and hole in clevis.

Employee was pulling miner cable out of roadway and fell, landing on his left arm.

Stepped on miner cable, it rolled and he fell. Twisted right ankle when he fell.

Employee was riding in a motor when it lost traction and hit a manbus causing the employee to his his knee against the frame.

A rock fell out of the top hitting the extended slider curtain knocking it down and the slider curtain struck employee on his right side.
Employee was hanging pipe and turned around and he was struck in the left eye with a plastic tie.

Pushing scoop with ram car. Ram car got on top of bumper and fell off. He jammed his head on canopy. Surgery required on neck.
After cleaning rockdust lines, employee pressurized the duster , the hose blew off and struck employee on right side. Hurt right wrist, arm and shoulder.
While getting off lo-trac, twisted his right ankle.

Walking up stairs. Back started hurting.
Employee was walking up belt line, got to header, tripped, injured left knee.
When employee pulled walk through door, it had pressure on it and he strained his low back. EMPLOYEE DID NOT START LOSING TIME UNTIL 4/28/2008 DUE TO SURGERY.
Employee was walking on 2nd floor of prep plant and a piece of the floor above fell on him. They were not working in that particular area. It was just a freak accident.
Employee was riding on a bus and the brakes failed. Employee was thrown out of bus. **Employee started losing time again on 2/19/09**
Employee was walking and backed up over a rail and turned his right ankle
Employee stepped on a flight in stageloader and felt pain in right ankle
Employee was pulling on pry bar. When something popped in middle of back. He started losing time 5/1/08.

Picking up structure, a rib roll pinned left ankle under structure and rock.

Employee was putting belt hangars up next to a cable and the steel caught his sleeve and mashed his finger on his left hand.
While cutting out at the tailgate, employee crawled over the gob on #144 Shield and when he got up he had pain in low back.
Employee had bar, prying switch and felt pop in right upper arm.
Stepped in a hole and sprain both knees.

Walking to manbus, employee stepped in a hole with left foot, he hyperextended his right foot and felt pain in the big toe.

Employee was picking up a 75# wrench. Felt pop in left shoulder.

Employee was jacking on wrecked tie car, when he let the jack down it him the employee on the left index finger **REPORTABLE ONLY DUE TO STITCHES**
Employee was taking panel off Miner. While turning bolt with wrench, the back of his hand struck the sharp edge of the metal cover plate. **REPORTABLE ONLY DUE TO STITCHES**
EE was reaching for an object and inadvertently grazed a piece of cut steel that resulted in a laceration of his left hand.
REPORTABLE ONLY. NO LOST TIME. While holding a curtain up, another employee accidently stapled his finger instead of the curtain. Fractured his left little finger.
Employee went through run through drop between #1 and #2 entry and a strip off. The curtain slapped him in the face and got mud and trash in right eye. ***REPORTABLE ONLY DUE TO CORNEAL ABRASION AND TREATMENT***
EMPLOYEE WAS ADJUSTING FORKS ON LULL LIFT, RIGHT FORK WAS STUCK, CAME LOOSE AND CAUGHT THUMB BETWEEN FORK AND MAST. ******DETERMINE TO BE REPORTABLE ONLY, NO TIME LOST******

Occupational Illness. Carpal Tunnel. Reported in June but had surgery 10/9/08.
This was not an accident. An employee came into the Preperation Plant Office and sat down in a chair and bent over. Another employee laid him onto the office floor. The employee passed away while being transported by helicopter to the hospital. It appears that he died from natural causes. We are still waiting for an official work from the Medical Examiner.
