IGNITION: The Miner was cutting a large pot that had fallen out of the mine roof in the middle of the No. 2 Entry Face, when the ignition occurred. The flame was on the operator side of the Miner Head. The flame was put out with one fire extinguisher.

IGNITION- As the electrician was burning weld from the H/G drum, a spark from the torch fell into a crack in the footwall and set a bleeder on fire. The flame lasted 6 seconds and was extinguished with water. No one was hurt nor was their any property damage incurred.

IGNITION: The Continuous Miner was cutting coal in the No. 1 Entry on No. 8 Section and while cutting close to the roof, a frictional methane ignition occurred. The ignition lasted approximately 2 minutes and was extinguished with 2 fire extinguishers and a water hose.

IGNITION: While cutting and cleaning up coal in the No. 3 Face on the No. 8 Section, a frictional gas ignition occurred. The flame was put out with a fire extinguisher and lasted approximately 20 seconds.

IGNITION. The Continuous Miner was cutting the bottom rock in the No. 3 Entry on the No. 4 Section on the curtain side of the Face when a frictional ignition occurred.

IGNITION. The Continuous Miner was cutting coal on the No. 8 Section in the cross cut between the No.2 & No.3 entry on the curtain side when they saw a flame on the mine floor. They put the flame out with a water hose and one fire ext.

IGNITION-- The Longwall Shearer Machine was trying to cut a large rock at No. 101 Shield when the H/G Shearer Opr. saw a flash around the drum of the Shearer. When the machine and drum was cut off the flash went out. It lasted approximately 30 seconds.

IGNITION--The Continuous Miner was cutting in the No 4 Entry. While cutting in the middle of the face, an ignition occurred. The ignition lasted less than a minute according to the Miner Opr. Used wash down hose and sprays to put fire out.

Cutting coal in the No. 1 Entry on No. 8 Section and while cutting coal on left sidt of entry, an ignition occurred. Ventilation was disrupted by the Ramcar just prior to the ignition. Fire was put out with a fire extinguisher. The ignition lasted approximately 30 seconds.

The continuous miner was cutting coal in the No. 1 Entry on the No. 8 Section and a frictional ignition occurred. It lasted approximately 30 to 60 seconds and was put out with a water hose.

Longwall shearer was mining at No. 17 shield when an ignition occurred under the panline at No. 8 shield.

The Joy Miner was cutting bottom in the No 3 Entry on No. 6 Section that was driving the South Entries when an ignition occurred in the bottom of the Face. It took approximately one minute to put the fire out. This was a frictional ignition.

Employee was checking a pump that was being lifted out of water and when he reached out to lift one end up to inspect he felt pain in lower back. (EE HAS BEEN WORKING SINCE DATE OF ACCIDENT THAT OCCURRED AT JWR #4 MINE ID 0101247 AND HAS BEEN TREATED STEADLY FOR THE INJURY. EE IS CURRENTLY AT JWR #7 MINE ID 0101401 AND DID NOT START LOSING TIME UNTIL 12/17/15 DUE TO SURGERY).

Employee was walking on rough surface in the dark, when he turned he twisted his right knee. However the employee did not start losing time until his Saturday Night Owl shift on 9/12/2008. The employee is scheduled for surgery later this week.

He was operating shuttlecar, loading behind continuous miner. Miner hit tail of sc. He hit his head in top of canopy. Strained back.

Employee had come-a-long hooked to a bracket on each side of tilt cylinder, holding it up to change out pins. Employee was using his finger as a line-up tool. The bracket broke and caught his fingers between the cylinder and hole in clevis.

This was not an accident. An employee came into the Preperation Plant Office and sat down in a chair and bent over. Another employee laid him onto the office floor. The employee passed away while being transported by helicopter to the hospital. It appears that he died from natural causes. We are still waiting for an official work from the Medical Examiner.
