AT 7 AM THE LOWER PART OF THE DEEP WELL PUMP #4 SHROUD WAS BEING LOWERED INTO THE HOLE. THE OTHER END WAS LOWERED TO CONNECT THE 2 PARTS TOGETHER. AT 705, A METHANE TEST WAS MADE & 0.0 WAS ALL THAT THE SPOTTER READ. AT THAT TIME THE PREP PLANT MECHANIC STARTED WELDING. THAT IS WHEN HE FELT HEAT ON HIS HAND. HE STOPPED, LOOKED AROUND & STARTED AGAIN. IGNITION OCCURRED.

As the #12 CM Miner received power via the cable, a splice blew out in the long crosscut between #2 and #3 place and ignited gas located in the top of the entry. The cable was hung up at the time in the roof area. No injuries or property damage was incurred.
EMPLOYEE'S HAD BEEN WELDING ON THE SERVICE SHAFT ELEVATOR DOWN INTO THE MINE WHEN HE NOTICED A SMALL FLAME ON THE NORTH SIDE OF THE ELEVATOR SHAFT. THE FLAME WAS APPROX. 1-12""WIDE & APPROX. A 1 AND 1/2""FLAME. A SPARK FROM THE BURNING & WELDING IGNITED A METHANE BLEEDER THAT WAS LOCATED APPROX. 1,048' DOWN THE SHAFT. THE FLAME LASTED APPROX. 1 MINUTE & WAS BLUE IN COLOR.
While burning bolts off the Bunker Belt Head Roller at U/G Prod. Shaft Pocket (with torch), sparks from metal that was being cut ignited methane on top of the coal that was located in pocket. Ignition lasted 20 to 30 seconds & flame was extinguished with water. There was only 0.1% methane found prior to & during burning of the bolts. No property damage or injuries were incurred.
On Friday, 5/27/05, water inflow into No. 5 Mine exceeded pumping capacity. No employees other than those involved in water pumping were allowed in the mine after the Friday Owl Shift. Pumping efforts continued until 8:15 AM on Monday, 5/30/05. All power into the mine and the 5-3 Fan were then shut off. The mine is currently idle.
Gas Ignition, conditions contributing unknown.
A brake solenoid grounded and caused the hoist to trip.
As the 7LS1 Joy Shearer was cutting coal on the #1 L/W, an ignition took place on the T/G Drum which was cutting bottom rock as they went toward the H/G. The flame lasted approximately 10 to 20 seconds and was extinguished with the water sprays and a fire extinguisher. There were no injuries or property damage.
As the 7LS1 Joy Shearer was cutting toward the H/G an ignition took place at the T/G Drum. The resulting flame burned for approximately 3 minutes in the bottom and was extinguished with water and a fire extinguisher. There were no injuries or property damage incurred.
As the 7LS1 Joy Shearer was traveling to the T/G, the H/G drum was cutting bottom and the T/G drum was cutting top. A frictional ignition occurred at #50 shield on the H/G drum that lasted approx 30-40 seconds. It was extinguished with water and a fire extinguisher. No injuries or property damage was incurred.
Employee was pulling a LW shield off the cage. Shield came off dollies and fell on employee. Lower left arm was amputated. Also, injuries to lungs.
A rock fell out of the top striking the employee on the right hand and third fingers.
Employee got off manbus to go to the wall, stepped on a rock and turned his ankle.
Employee was walking on the 2-east belt and stepped in a hole and twisted left ankle and hit left knee on rock.
WHILE INSTALLING A DRILL STEEL, IT SLIPPED OUT AND HIT EMPLOYEE ON THE RIGHT HAND.
EMPLOYEE HAD CUT CHAIN TO SEPARATE FROM REST OF CHAIN & BENT DOWN TO PULL CUT LINK AWAY & FELT PAIN IN HIS LOW BACK.

On 3/5/05 employee was setting props in old 3 track entry, he slipped on loose slag (gravel) and fell and hit on left hip. Employee started losing time again on 11/30/2005 due to increased pain and nerve blocks.
While carrying tools to tailgate, employee slipped on flight bar and twisted right knee. EE did not start losing time until 12/3/05 due to increased pain.
Employee was raising the bucket on a mucker to insert a metal bar under the bucket for repair work. The bar was not secured and struck him in the left jaw and left side of his head when the bucket was lowered.
While sweeping on 3-east belt a rock fell off belt hitting employee causing her to fall. To stop the fall employee grabbed for the belt rail felt pain in left shoulder.
WHILE DRAGGING A ROCKDUST HOSE, EMPLOYEE SLIPPED AND FELL. **EMPLOYEE DID NOT START LOSING TIME UNTIL 1/12/2006 DUE TO SURGERY.**
EMPLOYEE WENT TO END OF TRACK TO UNLOAD TRASH OUT OF SCOOP ON TO TRACK CAR. WAS STANDING BETWEEN TRACK & RIB, SCOOP PULLED UP TOWARDS CAR & HIT A CURVE RAIL THAT WAS AGAINST RIB. RAIL FELL HITTING EE IN THE LOWER BACK, LOWER LEFT LEG & ANKLE.
ROCK FELL OUT OF TOP HITTING EMPLOYEE IN THE HARD HAT, RIGHT ARM & RIGHT LEG.
Employee was changing shield hose and hit left arm on shield leg. He did not start missing work until 11-21-05.
Employee was using a manbus to move the water filter stand on the longwall. When the water filter stand was moved it tripped the employee and he fell landing on the water filter stand.
Removing old pipe above #1 and #2 primary sumps, he fell and hurt his left shoulder. Employee did not start losing time until 11/17/05.
While binding a trash tub, using a cheater pipe, pipe slipped off binder arm and struck him on his face and head. Broke nose, chipped tooth, sutures required inside bottom lip.
THE EMPLOYEE WAS TAKING DOWN MINER CABLE TO GET SLACK SO THE CONTINUOUS MINER COULD MOVE TO THE NO.1 ENTRY FROM THE NO.2 ENTRY WHEN THE EE FELT PAIN IN HIS LOWER BACK.
Employee was going thru brattice door, had hand in between door and jam and another employee ahead of him let the door slam on his left hand. Employee did not start losing time until 11/9/2005 due to results of MRI - torn ligaments.
The employee went to put a block on the brattice he was working on and felt pain in his left shoulder and the block fell and struck him on his left shoulder.
THE INJURED WORKER SLIPPED ON AN OILY SURFACE WHERE HE WAS STANDING TO REPLACE THE LEAKING SEAL AND TWISTED HIS LEFT KNEE. HE DID NOT REPORT THE INJURY FOR A FEW DAYS AS HE THOUGHT IT WAS A MINOR INJURY. AFTER DR VISIT & AND MRI IT WAS DETERMINED THAT SURGERY WOULD BE REQUIRED TO HIS KNEE. THIS WAS DETERMINED ON 11/3/05.
DID NOT START LOSING TIME UNTIL 11/7/05. While crossing over stageloader, got foot caught in hoses and twisted left knee.

EMPLOYEE WAS ON #110 SHIELD AND #109 WAS PULLING. EMPLOYEE LEANED OVER CABLE THINKING THE SHIELD WAS THROUGH PULLING AND HE WAS MASHED BETWEEN THE TOP OF THE CABLE TROLL AND THE BOTTOM OF THE SHIELD. (Misc Mail 6/6/06 #1-employee lost time again for one day on 6/1/06)
Employee was hanging curtain in #1 Place and pulled his shoulder. IT WAS NOT DETERMINED THAT EMPLOYEE HAD A ROTATOR CUFF TEAR UNTIL 9/1/05 DUE TO DR REFERRAL AND MRI.
Broke left ankle when he stepped in floc and slipped and fell.
Strained neck and upper back while riding bus. Bus ran into a motor and jarred his neck.
EMPLOYEE TURNED AND GOT RIGHT FOOT UNDER THE SCOOP BUCKET
At the bottom two employees were taking an old bottom gate down and when it came loose the gate was too much for the employees to hold onto. At the same time one of the employee's right foot was under it and mashed his right foot.
DID NOT START LOSING TIME UNTIL 10/06/05. While pulling cable for a belt move, tripped and fell straining abdomen. Caused hernia, surgery on 10/6/05.
#5 MINE EMPLOYEE: EMPLOYEE HURT LOWER BACK WHILE LIFTING TIMBERS. ***EMPLOYEE STARTED LOSING TIME 8/4/05 DUE TO INCREASED PAIN***
Employee was wedging motor from power pack with pry bar and he pulled his shoulder. Employee started losing time on August 31, 2005.
Concrete block fell on his left foot. While trying to get his foot out from under block, strained his left knee while twisting and pulling foot.
While removing radiator cap from hot radiator, steam blew out and burned his right forearm and he twisted his right knee while trying to move away. (8/4/05: Will need surgery on knee.)
The part employee was working with was on a pallet. The employee stepped on the pallet and it broke causing him to twist his knee. The employee started losing time due to his injury on August 3, 2005.
#5 mine ee: bus that ee was driving was struck from behind while swapping buses at end of track. ee did not start losing time until 8/1/05 due to increased pain.

EE attempted to move a large rock off the feeder. EE did not start losing time until 6/1/06 due to indending surgery.
EE was lifting drill overhead-dropped drill on knee.
EMPLOYEE WAS DIGGING OUT TO DRILL LEFT RIB LIFTER HOLE. A ROCK ROLLED OUT OF TOP HITTING EMPLOYEE ON THE BACK AND LEFT LEG. FOOT CAUGHT BETWEEN THE ROCK AND THE BOOM (FRACTURED ANKLE ON LEFT FOOT).
This is a #5 Mine employee temporarily assigned to #4 Mine. Employee was putting fork back on Lo Trac. Left little finger was caught between fork and top of fork support. Broke finger.
Employee reached, with left hand, and grabbed roof support on fork truck. He put his left foot up on forklift and his right foot slipped on a rock or small object causing him to fall. He broke his fall by grabbing with his right hand, pulling his back.
DID NOT START LOSING TIME UNTIL 8/17/05. While swinging pick, he hit his R.wrist on beam under rail. Was being treated for a sprain until the doctors discovered it was fractured on 8/16/05. Arm in cast.
DID NOT START LOSING TIME UNTIL 07/21/05. While climbing stairs, left knee popped and started hurting. Surgery required.
Injured employee lost his balance while reaching over from 3rd of 4th rung of step ladder. Injured employee fell and sprained his right ankle.
WHILE WALKING IN ON NEXT T/G ENTRY, EMPLOYEE STEPPED ON A BOARD WITH A NAIL IN IT
DID NOT START LOSING TIME UNTIL 6/15/05. While pulling shields over shear, rock was thrown from drum striking the back of his right calf, knocking him down and twisting his right knee. Surgery on knee.
Employee was walking beside supply car when he slipped on a rock an fell on right shoulder hurting his right shoulder and neck. (did not start missing work unitl 4-10-06) He last visit to a Medical facility on his shoulder and neck was on 1-5-06. He had a heart attack an had to undergo by pass surgery and on 4-10-06 had been released to undergo surgery to his neck.
EMPLOYEE WAS ON A LADDER CHECKING HOOK ON CHAIN HOIST. A RAMCAR CAME BY AND CAUGHT LEG OF THE LADDER KNOCKING EE ONTO THE TIRE OF THE RAMCAR.

THE EE WAS LIFTING MONORAIL CASTER UP TO INSTALL IT ON THE MONORAIL WHEN HE FELT A PAIN IN HIS LOWER BACK.
Fractured right tibia. Stepped off bolter, his foot got hung in curtain on footwall and he tripped, twisting his knee.
While hanging pipe, employee stepped on block and lost footing, landed on right arm hurt right elbow.

While jacking diesel motor back on the track, slipped and strained his lower back.

DID NOT START LOSING TIME UNTIL 5/31/05. Contusion of R. shoulder. While hanging miner cable on rib, rib rolled and hit his right shoulder and hand. Had shoulder surgery on 5/31/05. Worked until this time.
While getting off manbus to throw switch, EE twisted left knee.
EE WAS SETTING PROPS IN OLD 3 TRACK ENTRY. EE SLIPPED ON LOOSE SLAG (GRAVEL) AND FELL AND HIT ON LEFT HIP. EE DID NOT START LOSING TIME UNTIL 4/26/05 DUE TO INCREASED PAIN (HERNIATED DISC IN BACK).

Employee was removing a twisted drill steel from the drill pot when the drill steel struck him in the face under the left eye. (Laceration under left eye that required 7 sutures.)

Holding onto lifting head, it went down and mashed employee's right fingertips. Reportable only due to sutures: no lost time.

Rock fell from high rib or top and struck the ee in the lower left rib area while cleaning on 3 east belt.
Employee was cleaning trash box and caught his left little finger between the cover and the pipe.

Employee started missing work on May 5, 2006 not because of a serious injury but because of a illness (Carpal Tunnel) to hand. He had carpal tunnel surgery on May 8, 2006.
