Accident type, without injuries

ALLEGED IGNITION - There was a 103J order #8526271 issued on 12/24/13 for an alleged ignition on #1 Longwall. After an investigation by MSHA Inspectors, UMWA and Mine Operator it was determined that an ignition did not occur. We are submitting this only because a 103K order was issued and has not been vacated as of today and to be within the allowed 10 day reporting process.

Ignition happened at 11:45 pm. Employee stated that when he saw the ignition, he was bolting on the operator's side when it happened. After he handed to the fire extinguisher to another employee, he knocked the power. When it appeared to be out, he blew dust on it to make sure it didn't catch back up.

Roof fall occurred 4 crosscuts East off 4-7 fan shaft, due East of the 4-7 overcast line. The fall is approximately 80 ft. in length and 15 ft. wide. The roof fell out in a semi-circle shape along slickened surface leaving a 4-5 ft. brow along the length of the fall.

ROOF FALL: A roof fall occured on the No. 5 section above anchorage of the roofbolts that were installed. The roof fall was approx. 25 to 30 feet in length, 15 to 18 feet in width and 8 to 9 feet thick.

IGNITION - Occurred when the continuous miner was cutting the bottom rock on No. 11 section in No. 3 entry when a methane bleeder ignited on mine floor. Ignition lasted approx. 20 seconds and used 1 fire extinguisher and water hose.

The Cedar Creek Elevator was downed at approximately 8:45 pm due to a faulty door lock switch.

ROOF FALL: A roof fall occurred in the cross cut between #2 and #3 entry on the No. 6 section above anchorage of the roof bolts that were installed. The Roof fall was approx. 4 to 8 feet above the 6 foot fully grouted roof bolts that were installed and was approx. 115 feet long.

A fire was discovered in the engine compartment of #2 dozer. It lasted approximately 35 minutes.

The crane above the service hoist had an electrical problem in the drive unit. This unit is what allows you to let the hook of the crane up and down. We removed the hook and secured cables to the crane and put service cage back in operation.

The Cedar Creek Elevator was down from 3:58 am until 5:00 due to a faulty relay.

A fire occurred at 9:15 p.m. on the surface at the upper rock bin at head frame for rock belt PB8B, where a drive roller lagging and belt caught fire. The fire was extinguished using fire extinguishers.

Employee walked up to the 7-2C header to inspect wipers. The victim was on an elevated catwalk at discharge of the 7-2B conveyor belt to examine the belt wiper. He fell off the end of catwalk on 7-2B belt. Employee was caught between the skirt and the top belt resulting in a fatal injury.

Employee was installing support arm for canopy and while using come-a-longs the chain came out of the clevis allowing the canopy to fall hitting employee's finger causing the tip of finger to be cut off.

Employee was on ladder installing spads when he began to drill, a piece of rock broke and fell with the drill still in it pulling employees arm down with it causing pain in shoulder. Employee has been working since date of accident and has now been taken off work. Employee did not start losing time until 2/18/14.

Employee was jacking on wreck car on 8 section track when the jack mechanical slip causing employee to fall backwards injuring his right shoulder/elbow.

Employee entered through a man door right behind another employee and caught his left finger in the door. Employee did not seek medical treatment until 1/6/14 and was taken off work on 1/13/14 due to fracture to the finger resulting in surgery.

Employee was in the process of installing a rib pin at the #2 face when he bent the drill steel he was using, while trying to remove the bent drill steel from the drill pot the employee was struck in the head causing injury.

Employee climbed into a scoop and left knee locked up. Employee was sent for medical treatment and MRI which showed a bucket handle tear/lateral meniscus. His first date of loss is 12/13/13.

Employee stated while moving a pump, he felt a strain in his right upper arm. Employee was sent for treatment. Surgery was scheduled for 1/2/14. This is his first date of loss.

Employee was operating a locomotive when a collision occurred with the inby locomotive causing the operator to hit his left side in the operators compartment causing bruising to his left side. Employee went to the doctor the next day and was taken off work.

Employee walked around the Continuous Miner to operator side of miner when he heard the roof or rib pop, the employee turned to move and tripped and fell on right knee causing injury to knee. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOSING TIME UNTIL 12/13/13 DUE TO SURGERY ON KNEE.

Employee was working off side of stage loader taking down monorail, when a piece of coal rolled from rib striking his foot causing a stress fracture to the foot.

Employee was helping move a continuous miner, when he went to move the miner cable out of the roadway a small portion of the rib rolled out striking him on his left foot resulting in a strain and a contusion.

Employee was moving quickly from an area when he tripped and fell over a cat head while going by the power center straining his knee and leg muscle.

Employee was stepping down off of a rock truck when he felt pain in right knee. Employee has been working since date of accident. EMPLOYEE DID NOT START LOSING TIME UNTIL 12/6/13 DUE TO HAVING SURGERY ON HIS KNEE.

Employee was stacking roof plates on the roof bolter tray when a piece of rock fell striking employee on right hand causing a fracture.

Employee was using channel locks to pull belt up off of mine floor when he noticed pain in shoulder, he started training belt and when he swung a hammer he felt a pop in his shoulder causing a torn bicep.

Employee was carrying brattice blocks to the raise bore shaft when he slipped and twisted his left ankle resulting in a sprain to the ankle.

Employee was pulling the shuttle car with the scoop. The bolt heads that attached the bracket to the shuttle car, that was being pulled, sheared off resulting in one of the bolt heads flying back striking the employee at the top of the eye causing injury to the eye and face. **EE RTW on 10/2/13 but was taken back off work on 10/6/13 for additional treatment.**

Co-EE and IEE were carrying a nine foot track rail, when they threw the rail down, the rail bounced up and hit IEE on his right foot.

Employee was standing under a shield tip to hang mesh onto the shield canopy. When a coworker lowered the adjacent shield to allow the mesh to be hung, a rock slid off the lowered canopy and struck the employee on the right hip/back causing the employee to stumble backwards and hit the longwall face as he fell to the ground in the area between the pan line and the face.

Original accident: EE was pulling air doors open & felt a pop in both wrist & shoulders & neck. EE has been working since accident & was taken off 3/12/14 due to surgery on neck. As of 9/1/13 EE was working for North River Mine (0100759). North River Mine has since closed & EE was transferred to Mine #7 (0101401). I'm filing this report under the mine the acc originally occurred.

Employee felt pain in right shoulder while throwing timbers through door. Started losing time on 9/19/2013.

Employee was standing on opposite side of the Scoop from the employee using the hydraulic chain saw, when the chain on the saw broke and a small metal link from the chain struck the employee in the chin causing fracture and requiring sutures to his chin.

Over-exertion in lifting objects

Employee was pulling roof straps off the machine and stepping down from the roof bolter when he felt pain in the groin area. Employee was diagnosed with a hernia and had surgery to repair.

Employee complained with back pain after normal work duties. A MRI was performed which indicated degenerative disc damage. A previous MRI from 2005 showed many similarities. Employee did not lose time until 09/09/2013 due to doctor's exam.

Employee was shoveling and turned with the shovel full to walk to the belt, when he did his foot was stuck in muck and that is when he felt pain in his back. EMPLOYEE DID NOT START LOSING TIME UNTIL 9/10/13 DUE TO INCREASED PAIN.

Employee was starting to remove cutter shaft on longwall shear and when he took the bolts and snap ring off oil came out and burned both his lower arms and both hands.

Employee was going to replace a cut off valve for water on shield 115 when he turned a high pressure valve on and was struck in the face by the hose requiring sutures.

Employee was standing on left track of 475 Dozer, preparing to remove motor cover when he fell fracturing a bone in his shoulder area.

Employee was walking his fireboss rounds when he stepped in a hole and felt pain in lower back. EMPLOYEE HAS BEEN WORKING SINCE DATE OF ACCIDENT AND DID NOT START LOOSING TIME UNTIL 7/9/14 WHEN HE WENT OFF WORK DUE TO SURGERY.

Employee was putting together a double T outlet for 3"" air line, while tightening fittings with a pipe wrench he felt pain in right wrist resulting in a torn muscle.

Fall down stairs

Employee was walking down stairs from parking lot to bathhouse when he fell and when he reached out to catch himself he dislocated his left wrist.

Employee was reaching up hanging pinner cable when he felt pain in his shoulder. EMPLOYEE HAS BEEN WORKING SINCE THE ACCIDENT AND DID NOT START LOSING TIME UNTIL 10/1/13 DUE TO SURGERY.

Employee stated that he was pulling cable for the feeder move and injured his left arm. Started losing time on 1/2/2014.

Employee stopped his motor and when he stepped off the motor his right knee buckled under him causing possible tear in ACL.

Employee was hanging cable behind roof bolter when cable tighten jerking him, causing him to fall against the rib hitting knee. Employee has been treated for contusion to knee and has been working since date of accident. On 7/3/13 the employee saw a orthopedic specialist and was diagnosed with a fracture to left knee and has been taken off work.

Employee was helping put a supply car on the elevator and was standing on the elevator when the operator uncoupled the motor from the car and backed up, the supply car rolled back and the employee's foot was caught between the supply car and the shaft collar resulting in fracture to his right foot.

Employee stepped off front of Roof bolter to clean the dust hose when doing so, he stepped onto a rock twisting his right knee. EMPLOYEE HAS BEEN WORKING AND DID NOT START LOSING TIME UNTIL 5/11/13.

Employee was attempting to pull slack in the 1 1/4"" water line. As he took a step he felt something pop in his left calf. Employee went for treatment and was diagnosed with a torn gastrocnemius. First date of loss is 5/4/2013.

Employee was helping install the plow on stage loader. The plow was held on by 2 pair of comealongs and 2 bolts. As the was being lowered the 2 nuts on the bolts popped off allowing the plow to twist and hit the employee on the foot. Employee was standing behind the plow.

AMENDMENT - Employee hurt his lower back while lifting bags of sackrete from pallet that occurred on 6/1/1985 **(THIS IS AN AMEDMENT TO THE ACCIDENT THAT OCCURRED 6/1/1985 THAT HE LOST TIME ON AND IS NOT A NEW INJURY OR ACCIDENT. THE EMPLOYEE HAS HAD CONTINUING MEDICAL ISSUES SINCE THE 1985 ACCIDENT).

Employee was standing at the front of the roof bolter scaling down loose top when rock rolled off rib striking employee causing him to fall and twist left knee. Roof bolter was located in the # 4 entry. Torn ACL and meniscus. Referred to Dr.

Employee was walking by a supply car when he stepped on a rock spraining right knee with a possible meniscus tear.

Employee was walking down steps at 1 West header and she stepped off the last step slipped and turned her left foot and ankle.

Employee had thrown the track switch when the switch point rail and straight rail became bounded against bottom part of rails. The employee placed his left hand on the switch point rail to pick it up and the rail suddenly moved pinching the middle fingers of the left hand between the two track rails causing fracture to fingers.

Employee was walking through mud in tailgate carrying 6 to 7 cap boards. Supervisor instructed him to stop and he would help employee. Employee said no I got it. Employee took 2 more steps and fell forward putting all his weight on right knee that struck a timber in the mud. Doctor took employee off work the next day for a right knee sprain.

Employee was lifting, tugging and twisting a 58 HP pump to place for installment and felt pain in his right shoulder. He had surgery on 7/14/2014 and this would be his first date of loss as well.

Employee hit the LAS button on adjacent shield. Shield 78 did not move. Employee lifted curtain line and reached inside shield to move shield. The shield advanced catching his left foot between relay bar and pontoon. Shield had to be base lifted in order to remove his foot. The doctor took him off work.

Rock fell off the side of the shuttle car and struck employee on the top of the left foot. He was treated at DCH and taken off work.

Employee stated that while he was putting torches into the scoop bucket, he felt a pain in his belly. His first lost workday did not occur until 03/01/2013 due to surgery.

Employee was walking and tripped over a jumper air hose going to the ram car to jump off car. Employee was treated and was taken off work.

Employee trammed his diesel locomotive into a parked shield that was on another locomotive sitting on the track outby the Longwall recovery switch resulting in him sustaining fractured ribs.

Employee was walking down steps on 71 overcast on N-B belt. The handrail gave way causing employee's right knee to twist. Accident happened on 1/21/13. The first date of lost time is 11/7/2013.

Employee was taking out a 8"" metal check valve in water line when another 8""x8"" metal check valve fell hitting the employee on the top of his left foot causing a contusion.

Employee was riding in bus and his bus slid into another bus resulting in employee hurting his back but continued to work and continued back pain. The doctor took him off work starting 1/22/13. Investigation determined the rails had moisture on them from weather changing. All rails were examined and sand applied. DID NOT START LOOSING TIME UNTIL 1/22/13.

On E4 Longwall setup face, the canopy of shield 72 was under 73 side shield a T-Board was placed between the shields when pressurized the T-Board shattered in pieces when shield kicked into place and a piece of the wood struck employee in the mouth causing laceration to lip and broken tooth.

Employee was unloading power supply box from versa track into core box when he dropped on hand causing fracture and sutures to right hand fingers.

Employee stated that he pulled his shoulder out of socket. Treated by doctor. Employee has a rotator cuff tear and has a partial torn bicep tendon. His first date of loss is 4/29/2013.

REPORTABLE ONLY - Employee was unloading crib blocks and caught his index finger between the crib block and shield resulting in a fracture to his finger.

REPORTABLE ONLY: Employee was walking toward the headgate using the handrail to steady himself, when his hand slipped off rail and into another rail causing a fracture to the fifth metacarpal.

Employee was cutting strings off of a belt and the roller jerked the pliers out of his hand and pulled his left index finger into the roller. Employee was sent out for treatment. He received seven stitches.

REPORTABLE ONLY - Employee was helping take belt wiper from belt tool room to manbus when he dropped his side down on his middle finger and the tip of his finger was caught between the wiper and the manbus causing fracture to tip of finger.

Employee was cleaning out tray to stock roof bolts. He slipped and fell into the tray on the bolter causing a laceration to his chin. He was sent out for treatment and received sutures.

REPORTABLE ONLY: The roofbolter machine was backing up and the cable reel was engaged on when the cable tightened up hitting the employee (that was outby the machine) on the inside of the knee. THIS CAUSED A PARTIAL MCL TEAR THAT WAS DIAGNOSED ON 10/25/13 AFTER A MRI, RESULTING IN A REPORTABLE INJURY. Employee has been working since date of accident and is continuing to work.

REPORTABLE ONLY - Employee was trying to correct alignment of belt by striking the roller with a hammer, when he missed the roller and his thumb hit hanger bracket causing fracture and sutures to end of thumb.

REPORTABLE ONLY DUE TO SUTURES - Employee was operating the headgate drum of the shear and was coming toward the headgate when a rock was thrown form the drum striking his hand causing a cut between his thumb and index finger requiring sutures.

REPORTABLE ONLY - Employee was burning on head frame at 7-2 production shaft and material blew under his burning goggles into his eye. Employee received prescription eye drops and was released back to work.

Employee stated that he was using a pipe wrench to get a stuck drill steel from roof. He did go for treatment and received stitches.

Employee and coworker were loosening panel bolts on the shearer with a ratchet and socket. They were assisting one another so that the socket did not slip off the bolt head. When the bolt broke loose, the ratchet head pinched employee's right finger between ratchet head and shearer frame. Employee received 5 stitches on 7/22/13.

The employee was using a pusher to unstop the clogged material where he was installing a rib pin and while doing so he struck his hand against the drill steel causing a cut to his little finger. EMPLOYEE DID NOT GO TO THE DOCTOR UNTIL 8/5/13 AND WAS THEN DIAGNOSED WITH A FRACTURE TO THE TIP OF THE RIGHT HAND PINKY FINGER.

Employee stated that he was getting socket sets out of tool box when a set fell out of his hand and struck his finger. This caused a laceration that required sutures.

Employee was helping hoist a beam to upper work deck of the slope belt and he was looking down to see if beam was hung on lower platform, when the hoist turned striking employee on side of face resulting in a fractured nose.

Employee was attempting to remove a pin from 6B header and the pin was in a bind. When he was trying to get it out, the pin came around and struck the left eye. Went for treatment and received sutures.

REPORTABLE ONLY DUE TO CHIPPED TOOTH - Employee was installing two sticks of resin in the hole when a small piece of rock flaked off falling between the roof bolter canopy striking employee in mouth causing small chip on front tooth.

REPORTABLE ONLY DUE TO SUTURES - Employee was helping throw a rock in back of shuttle car, when the rock slipped causing employee's finger to get caught between the rock and the frame of the shuttle car causing laceration to finger.

Employee was washing top off with water hose and a piece of sharp rock fell striking him in the left lower arm. He went to DCH for treatment and received four sutures.

Employee was hanging a 2"" water line with a wire, he went to reposition himself and the pipe fell and hit him on top of the hat causing a tooth to break. He did not go get treatment until 5/21/2013.

Employee slipped/fell into structure. Rollers caught her sleeve and pulled her into the rollers causing her fingers to get caught between the rollers. She also bumped her head against the structure causing a black eye. Both fingers were broken and one received stitches.

Pulling belt on a conveyor with a winch cable. Cable was hung in a roller so tension was being let off cable to remove from roller, when tension was let off there was a recoil in the cable that caused it to jump out and it struck employee on the left cheek below his eye. First aid was given on site and then employee went to hospital for x-rays which came back negative.

REPORTABLE ONLY DUE TO SUTURES - Employee was throwing the track switch and caught his finger between the switch handle and the metal plate cutting the end of his finger requiring stitches.

Employee was tightening up a screw jack with a sledge hammer and it hit his left hand with the wooden handle against metal screw jack arm. He received treatment at DCH where he received four stitches.

While trimming a corner of belt skirt on feeder, knife slipped out of employee's hand cutting right lower arm. Employee was sent for treatment and received stitches.

Employee was taking a 2"" Aqua Mine pipe apart and the pipe had all pressure removed but it was installed over a small rise in the mine floor and when it came apart, it sprung up hitting him in the nose causing a fracture and sutures to his nose.

REPORTABLE ONLY DUE TO SUTURES: Employee was unloading 4"" pipe from one car to another car when his left hand finger got caught between the two causing a laceration to his finger.

Employee was attempting to install a drop board in the #4 entry. He put a drill steel into the pot and hit the drill guide lever to close the drill guides. His hand was not clear of the guides and he mashed his left fingers between drill guides. Treatment was needed. He received stitches.

REPORTABLE ONLY DUE TO SUTURES: Employee was using track roof bolter and adjusting the roof bolt plate and as he tried to flip the plate it caught his hand between the corner of the plate and the drill clamp causing a laceration to his left thumb.

Employee stated that he went up to the S-1 tailgate to monitor pumps. After he got through the man door at brattice 4, he tripped over track rails that had been pulled. He fell on his left hand trying to break his fall. He tried to treat at home. Pain and swelling caused him concern so he went to the doctor on 2/19/13. He has a fracture in his left ring finger.

Employee was standing facing the head gate talking to employee coming in on next shift when his foot was caught between the relay bar and pan causing fracture to foot.

Employee stated that he was using a pry bar when the bar slipped and struck employee in the left side of his bottom lip. This resulted in a laceration that required sutures.
