Accident type, without injuries

A roof fall has occurred in the intersection of Flat Creek Main #6 entry between spad #33+25 and #36+05. The fall measured 180 ft long, 20 ft wide, and 8 ft high.

A supply car caught on a beam as the service cage was being hoisted from the bottom. The car caused the cage to wedge in the shaft. The bottom of the service hoist was damaged.

The Cedar Creek Elevator was down from 1:40 am until 4:00 am due to motor brake release fault.

Lifting the longwall headgate cell plate, the lifting choker broke and allowed the unit to fall onto the cage floor. Cage floor was damaged. After damages were accessed support structure was repaired and cage floor was repaired.

The approach to the North side of the 4-5 fan shaft had a rock fall above anchorage. Area was dangered out and timbered.

A fire occurred in the engine compartment of a diesel lo trac. The fire burned for approximately 12 minutes.

A roof fall was discovered in Flat Creek Main return. It was approx 10' x 8' x 15'. Some of it was above the anchorage point.

The number 7 section, cutting in the number 3 face, had a methane ignition.

IGNITION: Occurred on 8 A belt at the tail piece, in the tail piece frame. Employee was burning holes in the header frame, while burning the holes, sparks went in between the bottom of the header frame causing a ignition. The flame lasted 10 seconds or less and was put out by a hand full of rock dust.

IGNITION - Occurred due to the connector insert breaking between the two drill steels used to drill an 8' deep hole for 8' long cable bolt. When the two drill steels were removed from hole methane being liberated ignited. Ignition lasted appox. 10-15 seconds and was extinguished with one fire extinguisher.

While attempting to cut rock from the roof to ramp up over an existing fault in the Number 2 entry, the extendable exhausting line curtain was being pressed against the rib rock protruding out from the rib. This condition caused the ventilation to be choked down making it difficult to ventilate the face.

IGNITION - occurred while burning the insert out of a miner bit lug when a spark ignited a methane bleeder on the inby rib. The flame burned 10 seconds and was extinguished with (1) one fire extinguisher.

Employee was trying to get cable out of the hook. The operator of the miner was moving miner and watching whisker close to rib not aware employee was trying to get cable out of hook. Employee's glove was caught in the Alpine hook and pulled his finger into it. The cable had a lot of tension on it and mashed the tip of his left ring finger.

Employee hit his elbow on a discharge water pipe, when he went to the doctor the next day it was determined he had a fracture to his left elbow.

Employee was climbing in the cab of a 50 Ton Crane when the grab iron broke lose causing employee to fall backwards and stumble, causing pain in lower back. EMPLOYEE HAS BEEN WORKING SINCE ACCIDENT AND DID NOT START LOSING TIME UNTIL 4/11/13. EMPLOYEE STARTED LOSING TIME AGAIN DUE TO SURGERY ON 2/19/2014 (THIS IS NOT A NEW ACCIDENT)

Over-exertion in lifting objects

Employee bent over to uncouple supply car and felt a sharp pain in his lower back. EMPLOYEE DID NOT START LOSING TIME UNTIL 12/14/12.

Employee was about 3-4' off the foot wall and jumped down to the foot wall into the water, he stepped down on a roof bolt covered by water, the pin head sprang up out of the water striking him above the left knee cutting his leg. Stitches were needed.

Employee was carrying a top roller when he slipped and started falling, dropping the top roller on his right foot causing a fracture.

Employee was working on ram car and slipped when getting off of ram car causing him to twist his right ankle. Diagnosis was a severe ankle sprain.

Two employees were carrying a Tram Sprocket to the Shear, when the sprocket slid off the bar they were using and hit the employee causing a contusion to his right foot.

Employee was helping move a cylinder from along the side of the feeder located in the bottom of the production shaft and he had his hand on the end of the cylinder and another employee pushed the cylinder catching employee's hand between the cyl. and frame of feeder causing a crushing injury to the end of his little finger on right hand.

Employee was stepping through man door stepped down wrong and twisted left foot. He has a fracture to his left pinky toe.

Employee stated that he was stepping over a retaining wall when he twisted his knee. An MRI showed a torn meniscus. His first lost work day did not occur until 10/29/2012.

Employee stepped off of bus to throw a switch and right knee twisted and she heard a pop. An MRI was done and it shown a torn meniscus. She has been taken off work and referred to ortho. Her first date of loss is 11/1/2012.

Employee was pulling hoses through brattice and a part of the plate that the sheave wheel was connected to broke off hitting employee in the back of leg, right calf, causing a contusion and bruising and started loosing time 10/19/12.

While moving a section feeder, the metal side bat wing broke off and landed on employees leg and ankle causing a fracture to his lower leg.

Employee parked this bus into S-3 switch and walked back to switch to throw straight. As he was walking back to the bus he tripped on a donut crib and fell on his right shoulder. Started losing time on 10/3/2012.

Employee was installing covers on headgate drive sprocket, when one of the covers fell down pinning his left ankle underneath the cover causing fracture to ankle.

Employee was riding on a motor, he raised up and turned to activate block light and hit his head on a 4 inch pvc pipe causing contusion and strain to head and neck.

Employee was picking up oxygen tank with cap and the cap came off when employee picked it up and it struck him in the mouth. Employee had surgery on 10/9/12 when he started losing time.

Employee was pulling rock dust hose that was tangled. As he was pulling the hose it caught on drive support and when it came loose, he slipped in wet rock, dust and mud injuring his lower back. His first date of loss is 9/13/2012.

Employee was pulling hoses and walking. He stepped on an uneven rock causing him to twist his left foot and ankle. He felt his ankle pop with a burning sensation. Started losing time on 8/21/2012.

Struck by falling object

Employee was assisting in the turning over of the stage loader drive assembly. Employee had his hand on the drive as it turned. The drive was approximately 3/4 ways turned over when the unbolted deck plate turned loose, striking employee's left middle finger.

While working on the lotrac, wrench slipped and struck employee's left wrist against the lotrac. Fracture ulna.

Employee sprain neck while riding out on a manbus. Employee has continued to work since accident until 8/16/12 when taken off work by the doctor.

While changing scoop battery, scoop arm (that holds battery) struck battery that was on the footwall pushing it on to the top of the employee's left foot. Employee's first day off was 8/9/2012.

Over-exertion in lifting objects

Employee was lifting slider rail with curtain from out of mud/water on footwall when he injured his right elbow. He has been working since injury up until he started losing time on 10/29/2012.

Employee stated that he was carrying a 5 gallon bucket of oil to dozer and felt a pain in his back. He went to the doctor a couple of times and was taken off of work on July 27, 2012.

Employee heard the left rib pop and side stepped to the right and tripped over rock and fell landing on right wrist and injuring it. Employee started losing time on 8/3/2012 and is scheduled for surgery. This interns last day with JWR is on 8/10/2012.

Employee was walking down steps when turn on landing and twisted left knee. Employee started losing time for the first time on 8/14/2012.

Belt crew was pulling slack out of the take up with a belt dog and belt winder, the belt dog slipped off causing employee to fall down on his knee, causing a fracture to leg just below his knee.

Employee was shoveling in production shaft when he got debris in his eye and it caused a scratch to his eye.

Employee stated while he was walking he stepped over a jack when his right knee buckled causing a sprain of the knee.

Employee stated while walking he stepped on a rock and rolled over his right ankle.

Employee stated he and another employee were carrying a top roller for belt structure when they tripped over a metal pin protruding out of the ground causing employee to injure his right hand when top roller fell on his hand.

Employee stated he and another employee were carrying a top roller for belt structure when they tripped over a metal pin protruding out of the ground causing employee to injure his right hand, thumb & forearm when top roller fell on his arm.

Employee was on platform bolter when he stepped down onto the mine floor, stepping on a T-board and twisted his left ankle.

A coal rib rolled out striking employee's lower leg and foot causing a hair line fracture above the ankle.

Top was hanging about 50' behind trailer and 50' or more behind line shields. Top bumped and broke and fell taking out supports on crib line striking employee in his ribs and head. The top took out 3 complete cribs and approximately 60 crib blocks. Blew crib block from 5' to 9' from where they were built.

Employee was lifting two bags of rockdust from the scoop to make room for a pallet of dust when he felt pain in his low back.

Struck against a moving object

Employee stated that the carrier he was on bumped another carrier which caused him to hit his head. This caused a laceration in the back of his head that required sutures.

Over-exertion in pulling or pushing objects

Employee had been welding on the canopy retainer rings on the shield job for North River. When employee moved the wire-welder, he felt a pain in his lower back and right shoulder.

Employee was stepping off of cage when he fell, landing on his left knee. Employee has been working since date of accident. EMPLOYEE DID NOT START LOSING TIME UNTIL 11/21/12 DUE TO SURGERY ON LEFT KNEE.

The tailpiece has a lot of rock that had fell off belt due to narrow belt and rib rolls - lot of water, causing employee to slip and sprain his right foot. Employee did not go for treatment until 5/17/2012 and it was determined that he had a hairline fracture. EMPLOYEE STARTED LOSING TIME 5/22/2012.

Employee stated he was carrying a bottom belt roller on his right shoulder. He started to walk with it when his foot was stuck in mud causing him to loose control of the roller. This caused employee to strain his right shoulder.

Employee had raised the forks and mask of the Mega Trac to see what was needed to repair or replace the lift jack. The employee positioned himself under the mask; the mask leaked down and pinned the employee between the mine floor and mask.

Employee stated he was helping move old belt structure thru a man door. Another employee slid a belt rail thru man door striking injured on shin below right knee.

Fall from machine

When getting out of the versa trac, the door came open quickly causing employee to fall landing on right arm and shoulder. EMPLOYEE HAS BEEN WORKING AND DID NOT START LOSING TIME UNTIL 5/17/12 DUE TO SURGERY.

Employee was standing outside of bus and took the parking brake off the bus. The bus then rolled 2 to 3 inches causing the seat to fall and smash employee's right ring and small finger. Employee had lacerations and needed stitches.

Employee stated that she was walking to her car when she slipped on uneven surface. Her first lost work day did not occur until 4/16/2012 due to surgery.

Employee stated he slipped and fell (he did not know what he fell on). He said he felt blood running down his left upper leg and went to call operator. He was walking into the crosscut on the offside of East B x-cut 27. He had a laceration and his leg required stitches.

Employee went to swing boom out to put in a rib pin when he was struck in the chin by an object; this then caused the employee to lose balance on canopy. The employee has laceration to chin and sprain left shoulder.

Employee got thumb caught between lifting bolt and miner head causing laceration and contusion to thumb.

Coal rolled out from rib striking employee on left lower leg causing a contusion. ****DID NOT START LOSING TIME UNTIL 3/28/12.****

Over-exertion in lifting objects

While lifting a 20 Ton jack the employee felt a pull in the left elbow. Employee has worked up until having surgery for a torn tendon 5/24/12.

While traveling in #181 man bus on the East main travelway the bus wrecked at bottom of the hill at cross cut 12. Employee had laceration on head and contusion of left hip.

Employee was cleaning work area from building brattice and tripped over work material causing him to fall and strike his right thigh on concrete blocks.

Employee was crawling through mandoor while a co-worker was holding the man door open. Co-worker accidentally lost hold of the mandoor causing it to slam down on employee's right lower leg causing a fracture.

Employee was hanging cable and stretched to put cable on j-hook and felt pain in his left shoulder. Employee started losing time for the first time on 12/30/2015.

Employee was washing off motor and bent over to spray bottom motor deck when he felt pain in his lower back.

Employee stated that he stepped off the back of the bolter to hang a curtain and when he stepped off, he stepped into a water hole and onto a rock rolling his left ankle.

Injured employee stated while helping another employee handle belt structure top rollers through a man door he strained his back.

While stacking props (round material) one rolled off striking employee in right knee.

Employee was pushing axle assembly over to remove axle from the locomotive. Axle was broken allowing motor to drop on the left foot fracturing it.

This employee started losing time again for this DOI 2/22/2007 on 2/22/2012. Employee injured his back when cleaning rib roll.

Employee stated that he was having a pain in his left shoulder after catching himself when he stumbled. He has had numerous shoulder surgeries and an MRI shows a mild metallic artifact within the greater tuberosity from prior surgeries. He did not lose time until 03/15/2012 due to surgery.

Did not start losing time until 2/22/2012. Employee was walking through double fly pads with arm load of T-boards and got tangled up and fell injuring his right knee and ankle.

The track that ends at the East B had a slick spot about 10 feet long. The operator yelled out to hold on but it was to late and the bus collided with the inby parked bus. Employee felt pain in back. Employee did not lose time until 2/4/2012.

The employee was rolling up miner cable when twisted to the left and felt a pop in lower back. ***Employee has been working since date of accident on restricted duty - was taken off work 2/18/12 for surgery***

Employee torqueing stem bolts into reamer body. The torque multiplier handle slipped out of employee's left hand striking his right hand across the knuckles, lacerating tendons on ring finger and little finger of right hand.

Employee was stacking fallen mix back on the lo-track when he felt a sharp pain in his low back. Employee did not lose time from work until 1/11/2012 for this injury.

Employee was washing shields down. Shear was tramming to tailgate. Rock was in bibby chain when shear was tramming. It pulled rock between chain and pan. Rock broke off, struck employee over the right eye. Stitches were required.

Employee was using rock bar to pry hollow shaft loose using both hands, when the shaft broke free the pry bar came up and his hand on the bar hit his tooth causing a crack in the tooth. REPORTABLE ONLY DUE TO CRACKED TOOTH.

Employee stated he was setting up a prop and the prop fell back against a rib pin, pinning right pinky finger against the rib. He received two stitches after his finger nail was removed.

Employee stated that he walked out of slope door walking out-by and air forced coal dust or dirt got into his right eye. He was sent out for treatment and received antibiotic ointment to go into his right eye.

After employee sharpened axe, he noticed axe head was loose. He secured the axe handle in vice grip and inserted two screws into the end of handle to tighten the hand to the axe head. When he loosed vise to remove the axe it slipped and he went to grab axe in his left hand causing a laceration that required sutures.

Miner was pulling removing rub board from 125 miner operator side. He was in the process of putting rub on top of miner. Rub board was placed on bolt sticking up and it flipped over and pinched miner' s right thumb causing a laceration. This required stitches. He missed following shift due to sleep deprivation per MD.

Employee was helping jack wrecked loco up. He was holding jack while another co-worker was jacking. When co-worker pushed down on jack handle hard, employee's finger was under it causing it to pinch and cut his right index finger. This required stitches. No lost time.

Employee was taking apart rock dust pipe and water line on belt entry on K-Panel of mine. Employee was walking inby in the belt entry when he tripped on a spreader bar and fell into a top roller frame that left a laceration on the shin of his right leg. Employee received four sutures.

REPORTABLE ONLY DUE TO STITCHES: Employee was turning Roof bolter machine into a cross cut and got close to the rib when a small portion of the rib rolled out striking employees hand against the machine causing a laceration requiring stitches.

Employee stated that he was moving mantrip when batteries shifted, catching his finger between frame of mantrip and batteries.

EE was bending and forming heavy gauge wire. The wire slipped and stuck in EE's hand, cutting him at the base of his left thumb.

Employee said he was preparing to wash lotrac and lifted the hood and then the hood fell on employee's left hand and cut his middle finger and ring finger. Employee was wearing gloves when accident occurred. Employee did receive stitches.

Employee stated that while changing a top frame, he pinched his finger between the frame and the belt structure.

Employee was helping fix a problem on the miner. He was resting his left hand on the top skid bar on the side of the deck pan when another employee closed the skid bar causing a laceration to the left pinky finger. Employee did receive sutures in his finger.

Employee stated that he was replacing a rubber hose. When he cut through the hose, the knife cut his arm. It required 4 sutures to close the laceration.

Employee was using a cheating pipe to break a motor bolt loose. The ratchet slipped and struck employee in the face causing a laceration that required stitches.

REPORTABLE ONLY DUE TO STITCHES: While drilling a hole for roof bolt a small rock came down the drill steel bounced and hit employee on the nose causing a laceration.

REPORTABLE ONLY DUE TO SUTURES - Employee was trying to move a rock at the longwall tailgate, when employee lost balance and fell, causing laceration to left middle finger requiring sutures.

REPORTABLE ONLY DUE TO BURN - NO TIME LOST. While using a torch and burning a bolt out of wiper shock a fire ignited from shock causing burn on face.

Employee went to throw switch on track, the throw handle was loose causing handle to come down onto his finger and right thumb. (SUTURES)

SUTURES - While unloading 4"" beams off flat car he went to set the beam down and caught finger between beam and rock in the foot wall.

Employee was using a pry bar when it slipped causing the employee to fall backwards and hit his head as his hard hat came off striking his head on a cross bar. Employee required stitches.

SUTURES - Was on pan line when bent over and removed hard hat for a second, when stood up struck power supply with top of his head. He required sutures.

SUTURES - Employee reached to turn on exhaust fan in the shop when his right hand struck a piece of metal sticking out in front of the switch.

Employee was sent a letter stating that he had evidence of pneumoconiosis. Employee, MIIN M00423747, had waived his rights as a Part 90 miner.
