IGNITION - Occurred in the headgate entry while burning bolts off the crusher element, which burned for 15 seconds and was put out by water and rockdust.

A roof fall has occurred in the intersection of Flat Creek 8 #2 entry in the intersection at spad #97+10. The fall measured 35 ft long by 28 ft wide by 5 1/2 ft high.

IGNITION - Welding on E-5 belt line at spad 24064 using torch to burn hole in some guarding and the flame was seen between support beam and mine floor. No injuries were incurred.

IGNITION: Miner Operator backed Miner out of face on No. 3 Section over to off curtain side to cut face. Miner approximately 20' outby and operator saw a blue flame on mine floor between cat track and pan. The flame was put out within 20 seconds. No injuries were incurred.

A roof fall occurred behind the wash down bay of the motor pit. It was 3 ft wide by 8 ft long by 8 ft high.

The slope hoist was down from 6:00 am on 8/14/2011 until 12:40 pm on 8/15/2011 due to the tachnometer on the drive.

A rock fall occurred on Flat Creek 6 Section. It was in the intersection of spad 7+32. It measured approximately 19 ft wide by 20 ft long by 7 ft high.

A roof fall has occurred in the intersection of Flat Creek Main entry #6 entry between spad #36+05 and #37+45. The fall measured 30 ft long by 20 ft wide by 10 ft thick approximately.

A roof fall occurred above the anchorage in the NO. 2 face on Section 7.A 103 order was issued.

The Cedar Creek Elevator was out of service from 8:00 pm until 11:30 pm due to a Field DC Contactor.

A methane ignition occurred at No. 45 shield on the face of the S-2 longwall panel (MMU 034-0), as the shear was cutting to the headgate. The ignition occurred on the tailgate drum. There were no injuries.

The mine roof in crosscut 25 on the east side of the North A track fell above the anchorage of the existing roof bolts. The affected area is approximately 25 feet in length and 18 feet in width.

IGNITION: Rock and coal were going through the No. 3 Feeder when a possible ignition occurred in the feeder chain area. A witness stated it lasted 2 seconds and went out by itself. Did not use anything to put it out.

IGNITION: Employee had started to weld on a CM miner head when another employee saw a flame on the mine floor. It appears that a methane bleeder was ignited from the sparks. The ignition was put out with water from a wash down hose located on the miner. The ignition occurred outby the face of No. 4 Section. No injuries were incurred.

IGNITION: An ignition occurred on the No. 4 Section in the crosscut, between the No. 3 and No. 4 Entries. The ignition extinguished itself apparently, in that the witness stated they saw a flash while cutting.

IGNITION: An ignition occured on the No. 1 Longwall at Shield 132 and lasted approximately one minute. The ignition was extinguished using water and two fire extinguishers. No injuries were incurred.

IGNITION: The Shearer was cutting coal and rock toward the H/G when an ignition occurred at No. 36 Shield around the T/G Drum. The ignition occurred on No. 1 Longwall. They used 3 fire extinguishers to put it out. No injuries were incurred.

IGNITION: A frictional ignition occurred while cutting the top at Shield 19 on #2 Longwall. It burned approximately 1 to 2 seconds and was put out with the drum water sprays. No injuries were incurred.

The mine roof on the No.3 section (MMU-009) fell above the anchorage point in the No.4 entry inby spad 28065 for approximately 90 feet and in the No. 3 entry inby spad 28064 for approximately 70 feet.

IGNITION: The miner was cutting bottom rock in the No. 1 Face on No. 7 Section when a frictional ignition occurred on the curtain line side on left of the Miner Head. Approximately 2 fire extinguishers and the water sprays put the ignition out. No injuries were incurred.

IGNITION: The Shearer was cutting at #10 Shield when a LW Helper saw and orange flame from around the HG drum. The flame lasted about 10 seconds and went out on its own. The shearer operator did not see the flame, only the LW Helper which only had 3 months experience.

IGNITION: Methane ignition on #2 LW at Shield 87 involving TG drum. Operator cutting coal headed to HG with 80% methane on monitor. Flame was orange and about 12 "" high and lasted approximately 2 to 5 minutes, it was extinguished with the water sprays on the TG drum. No injuries were incurred.

During the routine service hoist examination, the examiner discovered deterioration to the no. 2 tail rope.

IGNITION: A piece of metal became entangled in face chain on #2 LW causing a spark at joint in the panline w/shield 144 causing the ignition. The ignition was orange and blue in color and went out immediately after it ignited. It lasted only seconds and was approximately 12"" in height.

Employee was attempting to remove a man door from a pallet of block, in doing so while sliding the door off a stray block fell off striking employee on the foot.

Employee tripped and fell striking his face on the track.

A block fell striking employee in the left ankle.

Employee was taking chains off load on lowboy trailer. When he finished, he jumped down, off the trailer, and felt pain in heel of foot.

Employee was standing on right rib, behind miner, waiting on shuttle car when a rock fell from the top striking employee on right ear and shoulder.

Employee was helping with belt repair when rock fell out of top hitting employee in the head.

Employee was knocked down by shuttle car cable causing sutures and fractured elbow

Employee was letting his basket up in the bathhouse when something went in his eye. INCIDENT HAPPENED BEFORE START OF SHIFT BUT DID NOT ASK FOR MEDICAL TREATMENT UNTIL 4:00 A.M. (5 HRS INTO SHIFT).

Employee was cleaning off shield 120 with a shovel when a rock slid off canopy and fell in between shields 119 and 120 striking employees left knee and leg.

Employee stated that while talking with another employee, a shuttle car he was standing by was struck and pushed him into the rib. Employee's first lost work day was not until 11/04/2011.

Employee was struck by timber in his head causing his hard hat to go forward and press on his nose. This resulted in two sutures. Along with injuring his head, his neck, face and teeth were also injured.

Employee was helping pull an old push out blade out of ram car using a lo trac and nylon rope. The piece broke and struck the employee in the face. Employee first lost workday was 10/31/11.

Changing leg jack when leg jack slid against pontoon of 128 shield trapping right foot between pontoon and jack.

Stepped in a hole twisting knee while putting together joint of pipe. EMPLOYEE HAS BEEN WORKING UP TO DECEMBER 21,2011 WHEN EMPLOYEE WENT OFF FOR SURGERY.

While doing checks on supply motor employee walked from one side of motor to the other stepping on a small rock and twisted ankle.

While removing the lifting pin (80-100 lbs) from the shearer, employee dropped pin on left foot causing injury.

Accident 10/7/11 - Getting belt structure off of bottom belt he strained his lower back and R. leg. - Employee has been in physical therapy and working until January 2012 - January 6, 2012 employee was taken off work for lumbar block. Did not start losing time until 1/6/12.

Employee was removing a fuse from the panel, to check it. The fuse grounded to the panel and exploded. The panel was energized and the electrician was not wearing gloves. THIS EMPLOYEE WORKS AT THE CENTRAL SHOP BUT WAS INJURED AT CENTRAL SUPPLY.

Employee picked up top roller off manbus and felt pain in back. Employee has been working up to date 10/31/11. TAKEN OFF DUE TO SURGERY.

Employee was washing the slope belt when he went to turn off water and lost control of the hose with it striking his lower left leg.

Roof bolter was drilling a hole for roof bolt the drill steel broke and the employee went to pull the steel out of the hole, his body came in contact with the machine causing the drill steel to rotate catching his hand twisting it causing injury to middle finger.

Employee was working bent over. When he raised up, he felt a pain in his back.

Lifting top belt structure and felt pop in right arm and elbow.

EE didn't start missing time until 9-6-2011 and is scheduled for pre-op appointment on 9-12-2011 for surgery

Employee was walking down slope, slipped and fell causing compound fracture of right ankle

Employee was adjusting motor base while using pry bar slipped causing employee to fall on left ankle causing injury.

While working on belt line a guard over the wheel caught employee dragging over leg and pinching him between guard and the foot of take up.

While measuring the height of a head roller on the 3b installation area, the employee was struck by the portable head roller boom.

Employee was assisting in replacement of cross flow panels when employee lifted one side of panel felt pain in left forearm. **EMPLOYEE HAS WORKED FROM TIME OF INJURY UNTIL SURGERY 10/17/11 ON ULNAR NERVE IN ARM***

While lifting a rock with comealongs, a rock fell and struck employee on the hand

Employee was getting out of scoop backwards and stepped in a hole and twisted ankle.

While attempting to install rollers on belt employee strained his left shoulder while pulling the roller into position. DID NOT START LOSING TIME UNTIL 8/16/2011.

Employee at 1 West Header and slipped going down steps hitting his left elbow on the handrail.

**EMPLOYEE DID NOT START LOSING TIME UNTIL 9/15/11 DUE TO SURGERY** Employee stepped in hole twisting right knee while walking to man door.

Employee was detrashing and stepped in water hole on track tie and rolled his ankle.

Employee stated that he struck his head against T board. An MRI showed calcium spurring on several discs and disc protusion. Employee was placed off work on 8/5/2011 due to surgery.

EE tripped over rock while walking injuring his low back, right knee and right foot. He did not start losing time until 9/14/2011.

Employee was walking to kitchen/tool area when she slipped and fell causing injury to her right wrist.

Employee stated that he was tramming a scoop when he hit a hole and struck his head on the canopy.

Employee was drilling a rib pin and the steel bent and flipped back and pinned his hands under the tray above the controls.

A 1"" air hose came off muck pump and struck employee on the left side of his face and eye.

Employee mis-stepped over a 4"" piece of pipe to turn off water valve catching his heel and causing knee pain.

Employee was installing a tensioner cylinder in tailgate using a crane. He pushed on cylinder to line pinhole up and felt something in shoulder snap.

Employee helping move pump while using a bar to lift over the wall, the pump slid back down bar and struck employee on foot.

Employee was hooking two ends on air line pushing and strained his groin

Employee was in return behind 7/2 bunker in deep water and twisted his right knee

A gear boy was being sheaved from the stage loader with a steel cable. A chain broke holding sheave wheel causing cable to strike employee knocking him into a built crib.

Employee was changing top roller on belt and when he stepped off belt to ladder he slipped and hurt back groin and right upper leg

Employee slipped and fell when walking down slope and hurt his left knee

Employee stated that while working on the face at the new longwall, the gob came down and a piece struck his bicep. Employee first lost work day did not occur until 06/16/2011.

Employee was returning equipment and misstep on gob causing him to fall and twist his knee.

Employee was washing down belt with high pressure hose when the pressure increased causing the hose to move around with great force. Employee tried to control the hose and in doing so caused injury to the right wrist.

Employee riding on a manbus when another manbus lost traction and struck the manbus he was riding in.

Employee was walking slope belt and slipped straining left knee. EMPLOYEE DID NOT LOSE TIME UNTIL 8/8/2011 DUE TO INCREASED PAIN

Employee stated he was checking feeder on no. 7 section when he tripped on gob on bat wing causing him to fall and twist right knee.

Employee was cleaning canopy on #48 Shield and a piece of middle man slid down and broke off striking employee on the right lower leg.

Employee was helping to install a pressure gauge in 8"" aqua pipe when the pipe cracked and busted and knocked the employee to the ground causing multiple injuries including a laceration to the head.

Employee was drilling a hole in 8"" aqua mine pipe and the pipe cracked and busted knocking the employee off the ladder causing multiple injuries including a laceration to the head.

At first of shift, employee stepped in mud and twisted his left knee. Later he put weight on his left leg and felt his knee pop.

Employee shoveling belt when supervisor noticed employee in pain and lowered to his knees. Employee stated he had prior back issue and pain level was a 8 - he was removed from the mine for treatment.

The employee stepped through a man door and hit his head on a 2"" pipe. Employee didn't request treatment until next shift 5/11/2011.

Employee jumped off a piece of equipment and turned his ankle.

Employee stated that he was picking up a 5 gallon bucket of oil and felt a pop in his shoulder. Employee continued to work until the doctor placed him off work on 7/6/2011 due to a shoulder strain.

Employee strained his low back when he lifted a track jack.

Employee climbing down from bed truck. He slipped and landed in large gravel and twisted left knee. Started losing time May 6, 2011.

EE picked up box and felt a pop in his shoulder. EE started losing time on 5/12/11.

Employee hurt his knee while walking underground.

Employee's hand was struck by a rock on the spill plate on the pan line.

Employee walking across bathhouse and hit his toe on a bench. EMPLOYEE DID NOT START LOSING TIME UNTIL 7/25/2011 DUE TO SURGERY

Employee was holding a t-board against coupling while trying to place together pipe when the t-board slipped and hit employee in the ear.

ACCIDENT - Employee went to pick up a jack from manbus with hand and felt pain. employee saw dr. and was advised he needed surgery. Employee elected to put off surgery to later date and has been working up to this date. SURGERY - Employee had surgery 12/29/11.

Employee was involved in manbus accident that caused injury to the employee's neck and head.

Employee was involved in man bus accident that caused injury to the employee's right ankle/foot.

Employee involved in man bus wreck 3/22/2011 but didn't request treatment until 3/25/2011 stating pain in left knee and lower back. Employee worked until 3/31 and then was took off work due to work restrictions - surgery date 4/14/2011.

Employee shoveling under 4C belt A frame. Employee was lifting shovel over head to place material on belt when she felt/heard a popping in her shoulder.

Employee riding on a bus that wrecked, causing him to hurt his back. EMPLOYEE DID NOT START LOSING TIME UNTIL 7/23/2011 DUE TO INCREASED PAIN REQUIRING EPIDURAL BLOCKS.

Employee was shoveling belt when his shovel connected with a rock and he felt a pain in his right shoulder. Employee initially returned to work after accident until his surgery date of 4/21/2011.

#5 Mine Accident: Employee pulled his shoulder lifting an outside light fixture EMPLOYEE DID NOT START LOSING TIME UNTIL 7/21/2011 DUE TO SURGERY.

Employee was moving curtain to level footwall to start brattice and stepped on a nail in an old pallet.

Employee was walking around the boom of the miner and turned his right ankle.

Employee was raising canopy up to top and rock fell flipping back under canopy and struck employee in head, fracture to nose.

Employee was getting out of the bus and turned his right ankle. Employee did not lose time until 2/7/11 and returned to work on 2/11/11.

Debris fell from raise bore shaft creating massive air rush knocking employees to ground and assaulting them with particles of rock and mud.

When cutting out at tailgate shearer drum threw a rock striking employee on the right foot.

Employee was walking down walkway. He slipped and caught himself with his hands and hurt his left shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 1/31/2011 DUE TO INCREASED PAIN AND PENDING SURGERY.

Employee hanging water hose and stepped off ladder onto a rock causing him to twist his ankle.

Area of walkway had two timbers covered in dust that was extended under the track. Employee tripped over the first timber and struck his knee on a impacted rail.

Employee was walking with a slider rail and rolled his ankle while stepping over a block.

Employee was hanging miner cable on hooks during a powermove and sprained his ankle while trying to pick up more cable. Employee off work until 1/28 (working two days) and now is back off work until further notification

Employee was cutting jacket back on cable when the knife slipped and cut his left hand.

Employee was hanging cable and the cat heads fell out and struck the employee on the left shoulder. EMPLOYEE DID NOT START LOSING TIME UNTIL 1/28/2011 DUE TO INCREASED PAIN AND NO USE OF LEFT ARM.

Employee was working at the V-Wiper and turned and tripped over a block and fell twisting his ankle.

Employee was driving ramcar and hit a rock causing his knees to hit the dash and also jarred his knees. EMPLOYEE DID NOT LOSE TIME UNTIL 5/5/2011 ON HIS LEFT KNEE.

Employee was holding tee board on top of crib when employee using hammer let the hammer slip out and strike employee's right middle finger.

Employee was setting block on top of the wall and it came loose as he was lining it up. It fell striking his left thumb and hand. Stitches were required.

Employee stated that he was removing cathead when he struck himself in the mouth.

Employee was using a wrench to loosen a bolt on metal guard when the wrench slipped and employee cut his finger.

Trying to straighten hook when channel locks slipped striking employee in mouth. (SUTURES)

Employee stated that he was traveling on a rover when he ran into a motor.

While going through mine door the door closed on fingers cutting them. (SUTURES)

Installing a drive shaft and caught L finger between shaft and pinion. REQUIRED SUTURES.

Working at prep plant employee placed foot on overflow box which gave away causing him to cut lower leg against metal from box. EMPLOYEE RECEIVED SUTURES.

Reportable Employee was cutting belt with belt knife and cut his right upper leg requiring stitches.

While putting a frost pack kit through brattice door, cut right lower arm on sharp edge of door latch.

Employee stated that he was stepping down when all of his weight shifted to the edge of his foot.

Employee struck by plug while helping pull a stable from a pressured shield.

REPORTABLE ONLY : Employee was pulling curtain thru door and cut his arm on the hinge/latch.

#5 Mine Accident---REPORTABLE ONLY: Employee was trying to remove a dart valve from a float cell discharge box the valve stem pinched his hand between right thumb and index finger.

REPORTABLE Reached down to pick up the life line and cut hand on piece of banding.

Removing old steel from shaft and hoisting to mine level. While pulling old steel back on floor, a piece cut injured employee's left wrist.

Employee stated that he was changing out the blade on his utility knife when his hand slipped and cut his right index finger.

REPORTABLE ONLY DUE TO FRACTURE AND SUTURE. Employee was cleaning shields off and a rock fell and struck him on the left hand causing laceration and fracture to middle finger.

Employee stated that he was going through a mandoor when he cut his knee on a piece of metal.

REPORTABLE ONLY NO TIME LOST Employee was at the bretby chain and caught his finger in the chain mashing and cutting his finger requiring stitches.

Employee was cleaning the tailpiece when the roof bolt came over the 8-B head with a bounce it hit the employee in her forehead.

REPORTABLE ONLY DUE TO STITCHES: Employee was cutting rockdust pipe and the locking device on the knife failed and he cut his finger.

Employee was standing in the center area along side of car loaded with pipe when the pipe rolled off of the car and employee tried to hold pipe back injuring his fingers.

Employee cut his arm on the man door latch while moving a sheet of metal through door. He and another employee were carrying the metal thru the door when the other employee dropped his end causing employee to hit the door latch.

REPORTABLE ONLY DUE TO STITCHES: Employee was pulling pallets out of mud and a metal strap cut his hand

Employee throwing CM cable to the side and felt pain in his side.

Employee was taking hoses loose from the shield when employee was distracted by a mule coming down the face striking his left hand with a hammer.

REPORTABLE ONLY DUE TO STITCHES: Employee had finished pinning and was under supported top and lowered the canopy to allow for advancement and a rock slid off the canopy and struck him on the right side neck.

EE was heating and bending metal on the pan section so it could be bent back into place.

EE was cleaning weld splatter from inside the electrical panel with a grinder. The grinder slipped and struck back of right hand. Employee was wearing leather driving gloves. He was leaning deep inside the panel cleaning the back. The grinder was dropped when it cut his hand.

While placing track jack back on bus employee got her right ring finger caught between bus and jack.

REPORTABLE ONLY DUE TO STITCHES:Employee was cutting a zip tie and cut his left hand.

REPORTABLE ONLY DUE TO STITCHES: Employee was shoveling on the slope and her hand contacted the top belt roller, caught thumb between the belt and roller.

Unloading track rails and mashed right ring finger between pry bar and rail. Sutures required. No days away from work.
