IGNITION - Occurred in the headgate entry while burning bolts off the crusher element, which burned for 15 seconds and was put out by water and rockdust.

A roof fall has occurred in the intersection of Flat Creek 8 #2 entry in the intersection at spad #97+10. The fall measured 35 ft long by 28 ft wide by 5 1/2 ft high.

IGNITION - Welding on E-5 belt line at spad 24064 using torch to burn hole in some guarding and the flame was seen between support beam and mine floor. No injuries were incurred.

IGNITION: Miner Operator backed Miner out of face on No. 3 Section over to off curtain side to cut face. Miner approximately 20' outby and operator saw a blue flame on mine floor between cat track and pan. The flame was put out within 20 seconds. No injuries were incurred.

A rock fall occurred on Flat Creek 6 Section. It was in the intersection of spad 7+32. It measured approximately 19 ft wide by 20 ft long by 7 ft high.

A roof fall has occurred in the intersection of Flat Creek Main entry #6 entry between spad #36+05 and #37+45. The fall measured 30 ft long by 20 ft wide by 10 ft thick approximately.

A methane ignition occurred at No. 45 shield on the face of the S-2 longwall panel (MMU 034-0), as the shear was cutting to the headgate. The ignition occurred on the tailgate drum. There were no injuries.

The mine roof in crosscut 25 on the east side of the North A track fell above the anchorage of the existing roof bolts. The affected area is approximately 25 feet in length and 18 feet in width.

IGNITION: Rock and coal were going through the No. 3 Feeder when a possible ignition occurred in the feeder chain area. A witness stated it lasted 2 seconds and went out by itself. Did not use anything to put it out.

IGNITION: Employee had started to weld on a CM miner head when another employee saw a flame on the mine floor. It appears that a methane bleeder was ignited from the sparks. The ignition was put out with water from a wash down hose located on the miner. The ignition occurred outby the face of No. 4 Section. No injuries were incurred.

IGNITION: An ignition occurred on the No. 4 Section in the crosscut, between the No. 3 and No. 4 Entries. The ignition extinguished itself apparently, in that the witness stated they saw a flash while cutting.

IGNITION: An ignition occured on the No. 1 Longwall at Shield 132 and lasted approximately one minute. The ignition was extinguished using water and two fire extinguishers. No injuries were incurred.

IGNITION: The Shearer was cutting coal and rock toward the H/G when an ignition occurred at No. 36 Shield around the T/G Drum. The ignition occurred on No. 1 Longwall. They used 3 fire extinguishers to put it out. No injuries were incurred.

IGNITION: A frictional ignition occurred while cutting the top at Shield 19 on #2 Longwall. It burned approximately 1 to 2 seconds and was put out with the drum water sprays. No injuries were incurred.

The mine roof on the No.3 section (MMU-009) fell above the anchorage point in the No.4 entry inby spad 28065 for approximately 90 feet and in the No. 3 entry inby spad 28064 for approximately 70 feet.

IGNITION: The miner was cutting bottom rock in the No. 1 Face on No. 7 Section when a frictional ignition occurred on the curtain line side on left of the Miner Head. Approximately 2 fire extinguishers and the water sprays put the ignition out. No injuries were incurred.

IGNITION: The Shearer was cutting at #10 Shield when a LW Helper saw and orange flame from around the HG drum. The flame lasted about 10 seconds and went out on its own. The shearer operator did not see the flame, only the LW Helper which only had 3 months experience.

IGNITION: Methane ignition on #2 LW at Shield 87 involving TG drum. Operator cutting coal headed to HG with 80% methane on monitor. Flame was orange and about 12 "" high and lasted approximately 2 to 5 minutes, it was extinguished with the water sprays on the TG drum. No injuries were incurred.

During the routine service hoist examination, the examiner discovered deterioration to the no. 2 tail rope.

IGNITION: A piece of metal became entangled in face chain on #2 LW causing a spark at joint in the panline w/shield 144 causing the ignition. The ignition was orange and blue in color and went out immediately after it ignited. It lasted only seconds and was approximately 12"" in height.

Employee was attempting to remove a man door from a pallet of block, in doing so while sliding the door off a stray block fell off striking employee on the foot.

Stepped in a hole twisting knee while putting together joint of pipe. EMPLOYEE HAS BEEN WORKING UP TO DECEMBER 21,2011 WHEN EMPLOYEE WENT OFF FOR SURGERY.

While doing checks on supply motor employee walked from one side of motor to the other stepping on a small rock and twisted ankle.

While removing the lifting pin (80-100 lbs) from the shearer, employee dropped pin on left foot causing injury.

Accident 10/7/11 - Getting belt structure off of bottom belt he strained his lower back and R. leg. - Employee has been in physical therapy and working until January 2012 - January 6, 2012 employee was taken off work for lumbar block. Did not start losing time until 1/6/12.

Contact with electrical current

Employee was removing a fuse from the panel, to check it. The fuse grounded to the panel and exploded. The panel was energized and the electrician was not wearing gloves. THIS EMPLOYEE WORKS AT THE CENTRAL SHOP BUT WAS INJURED AT CENTRAL SUPPLY.

Employee picked up top roller off manbus and felt pain in back. Employee has been working up to date 10/31/11. TAKEN OFF DUE TO SURGERY.

Employee was washing the slope belt when he went to turn off water and lost control of the hose with it striking his lower left leg.

Roof bolter was drilling a hole for roof bolt the drill steel broke and the employee went to pull the steel out of the hole, his body came in contact with the machine causing the drill steel to rotate catching his hand twisting it causing injury to middle finger.

Employee helping move pump while using a bar to lift over the wall, the pump slid back down bar and struck employee on foot.

A gear boy was being sheaved from the stage loader with a steel cable. A chain broke holding sheave wheel causing cable to strike employee knocking him into a built crib.

Employee was changing top roller on belt and when he stepped off belt to ladder he slipped and hurt back groin and right upper leg

Employee stated that while working on the face at the new longwall, the gob came down and a piece struck his bicep. Employee first lost work day did not occur until 06/16/2011.

Area of walkway had two timbers covered in dust that was extended under the track. Employee tripped over the first timber and struck his knee on a impacted rail.
