Employee slipped while exiting cab and fell to the ground, injuring his right arm. Injury was not recordable at that time. Recent medical evaluation led to surgery 5/12/04.

Employee tried to strike a piece of the steering arm of a haul truck in place with a hammer. Employee missed his target and hit his left middle finger. Examination by a doctor revealed a fracture in his finger.
Employee hit his elbow against a piece of angle iron, causing it to bruise. Employee returned to work that day at full duty. On 10/11/04, employee visited the doctor because the swelling had not decreased in his elbow, and the doctor made an incision to drain excess fluid. Employee returned to modified duty on 10/12/04.
Employee strained back while bending over to pick up a fuel hose.
Employee felt abdominal pain when performing maintenance on a conveyor belt. Employee returned to work with restrictions.
Employee was changing out a conveyor ider when he slipped and fell. He was examined by paramedics on 5/25, and it was declared a first aid. Employee visited hospital on 5/28/04 and was placed on medication, making this a recordable incident. He was then put on restricted duty on 6/10/04 by our workers comp doctor.
Employee was pulling on a handle of a wrench and felt pain in his rib cage. He reported it to his foreman that night, but didn't seek medical attention until March 22, 2004.
EMPLOYEE PINCHED LITTLE LEFT FINGERTIP WHILE SETTING A CONCRETE BLOCK INTO PLACE.
EMPLOYEE EXITED PICKUP, CLOSED DOOR, AND TURNED TO GO TO THE OFFICE. HIS RIGHT FOOT SLIPPED & CAUGHT, CAUSING ANKLE PAIN.
Employee was using a hammer when he struck his finger. The finger was resting against a piece of angle iron when it was hit. The employee received three stitches in his finger and returned to work the next work day.
Employee was stung by a bee.
Employee caught his hand between a trailer edge and a piece of material that he was trying to put back in place. He was taken to the COMP doctor and five stitches were required to close the wound.
Employee was stung by a wasp while adjusting the cap of an oxygen bottle.
Employee alleges he hit his head on the top of the loader cab on 4/14/04. The injury was not recordable at that time. Employee returned to physician on 5/13/04 claiming that injury was not healing, and was prescribed physical therapy at that time.
EE WAS WELDING AND A SPARK ENTERED HIS CLOTHING CAUSING A BURN TO HIS ARM.
EE STARTED DOWN OFF BOTTOM STEP OF LADDER AND RIGHT FOOT SLIPPED, CAUSING A STRAINED KNEE. EE EXAMINED BY DR AND RETURN TO WORK. EE WENT BACK TO DR 2-9-04 AND WAS PRESCRIBED PHYSICAL THERAPY.
