Over-exertion in lifting objects

Employee picked up a pail containing a rock sample and strained his back.

While operating a loader employee felt pain in right groin area.

Scale clerk was operating the 980G loader. While navigating a decline, the loader began to pick up speed. The operator panicked & could not locate brake pedal. While attempting to steer thru a curve, the operator steered too far to the right which deflected the loader to the left. Left front tire hit a soft low spot overturning loader into a settlement pond.

Struck by flying object

Employee was grinding on a crusher liner and some debris flew into his eye.

Fall to the walkway or working surface

While investigating a problem with the pit belt beneath the 1 Jaw Crusher, employee tripped over a rock and fell to the ground.

Caught in, under or between a moving and a stationary object

(Note: the shift starts 6:00 pm through 6:00 am the following day). While removing a rock jammed between the skirt boards, the rock came loose and the bar moved unexpectedly pinching the employee's finger between the bar and the side skirt board.
