Over-exertion (Not Elsewhere Classified)

Employee was working at the Secondary Plant on Station #2. They were sliding screens out. He was trying to lift himself up on to the end of the 2nd deck so he could help get the screens out. When putting all his weight on his arms, his right shoulder gave away and popped out of place.

Over-exertion in lifting objects

WHILE LIFTING THE PUMP/FILTER UNIT FROM ONE PORTABLE FUEL TANK TO ANOTHER THE EMPLOYEE FELT HE PULLED SOMETHING IN HIS BACK. THE EMPLOYEE CONTINUED TO HIS WORK DRIVING A TRUCK AROUND 0830 THAT DAY THE EMPLOYEE REPORTED THAT HIS LEG WAS GETTING NUMB AND HE WAS TRANSPORTED TO THE LOCAL CLINIC/HOSPITAL COR CARE.

Over-exertion in pulling or pushing objects

Employee using a 2"" water hose - Pulled hose to remove a kink from it and the released energy from the hose knocked employee off balance causing her to fall.

Over-exertion in lifting objects

Employee handling an approximately 50 lb. rock sample in a 5 gallon bucket - Employee set the sample down on the floor and said his back was hurting.

Struck against stationary object

Employee was underneath a truck making repairs and his hard hat fell off - when employee reached to retrieve his hard hat he bumped his head, causing a small laceration to his head.
