Removing shims from cone crusher, finger was caught and the tip was partically amputated.

Employee was lifting rocks and sustained a hernia.

Struck by falling object

Performing maintenance on a cone crusher. The head was being lifted, a weld broke causing the cone to fall and strike the employee's leg.
