While retrieving samples from quarry face, employee was struck on left shoulder by a rock, resulting in fractures.

Fall to the walkway or working surface

Employee slipped on ice in the parking lot and fell on elbows and back - Received a fracture to right clavicle bone.

Employee was walking on uneven surface to his vehicle, when he stepped on a rock and turned/sprained right ankle.

Employee was standing on dozer track, fueling dozer - Hand slipped from grab rail, employee fell backwards to the ground, bruising right heel.
