Fall to the walkway or working surface

The employee was walking in the plant, going to measure Silo 58, when he stepped on an icy area, slipped and fell on the ground.

Over-exertion in lifting objects

The employee was removing a clogged 6"" line from the Fast Vac (vacuum truck). While lifting on the line, the employee felt a sudden pain in the lower right side of his abdomen that traveled down to the groin area. Note: Injury became a lost time 6/1/05 due to hernia surgery on 6/1/05.

Absorption of radiations, caustics, toxic and noxious substances

The employee was opening Elevator A boot door to complete a mechanical repair, when fine dust material flushed out of the door and entered boot, causing a burn to the left foot/ankle.

Over-exertion (Not Elsewhere Classified)

The employee was working on the bucket elevator. While lifting bucket elevator with chain fall the employee allegedly felt a sharp pain in his right shoulder.

The employee was helping to remove the 1935 drag chain. Not realizing the chain was caught on a piece of angle iron, the employee looked into the opening on the west end of the Polycom to see what the chain was catching on. As the employee did this he leaned his hand on the chain and it came loose, striking his right thumb.

Caught in, under or between (Not Elsewhere Classified)

The employee was changing the hammer vavle on the south line. The valve spun and the employee reached out to grab it - it came to a stop against the pipe wedging the employees finger, causing a laceration to left index finger.

Struck against stationary object

The employee was breaking open a cement bag, when his arm hit a sharp edge on the saddle that the bags sit in when loading - causing a laceration to his lower right arm.

The employee was cutting tubing for air conditioner at the quarry crusher, when a foreign object went into employee's right eye.

Fall from machine

EE was opening railcar covers for loading. As EE traveled length of the RR car, lanyard began to extend downward. Lanyard got stuck at attachment point of cable. Lanyard began to deploy. Once fully deployed, lanyard became taut, suddenly stopping EE as he traveled from one RR car to the next, causing EE to lose balance and fall (fell to pavement next to railcars).

Absorption of radiations, caustics, toxic and noxious substances

The employee was cleaning up dust in the elevator pit. While cleaning, some of the dust got into his boot (the employee was wearing half socks), causing a burn to the inside of the right ankle.
