Struck by powered moving object

Employee was walking near Gypsum Storage Building to return to the Finish Mill Building. Employee may have been struck by a piece of mobile equipment that was working in the area.

Over-exertion in pulling or pushing objects

While the employee was loosening a 4"" pipe cap with a 3' pipe wrench he felt pulling sensation in his back that caused a strain.

Struck against stationary object

While employee was kneeling on wooden floor assembling a partition, an apparent sliver entered his right knee, which later that night caused swelling and pain.

Over-exertion (Not Elsewhere Classified)

The employee was walking down a set of stairs, as he stepped out onto the main floor he stepped in wet mud and twisted his right knee.

Struck against stationary object

While disassembling a FK Compressor oil bath filter, employee caught his right wrist on the wire mesh of the filter causing a laceration to his right wrist, resulting in sutures being needed.
