Over-exertion in pulling or pushing objects

The employee was performing his regular duties when he felt a twinge in his left shoulder.

The employee was walking down the steps from the parking lot to the locker room building. It was raining heavily and the employee's left foot slipped off the wet bottom step and he twisted to catch his balance.

Caught in, under or between a moving and a stationary object

The employee was relocating an Acetylene bottle when his left hand became pinched between the elevator door and the Acetylene bottle, causing a contusion to his left hand.

The employee was replacing Roller Mill Roll bearing cover; weld on pick eye broke and caused cover to strike employee on left forehead, causing a laceration and contusion to forehead.

The employee was removing a temporary cable from under the kiln. The employee twisted and felt a pain in his back.

The employee was dumping cement bags into a hopper. While dumping the bags the employee strained his left wrist and forearm.

The employee got out of his truck to walk down the road to the Quarry. The road was icy, and the employee fell; resulting in pain to neck, lower back right shoulder and elbow.

Fall onto or against objects

Employee was walking to shower room - caught shoe on seam in concrete- tripped and dropped glass dish he was carrying. Employee fell and left hand hit the broken glass on the ground, causing a laceration to his left hand.

The employee was grinding welds in the kiln. The grinder caught his left hand, causing a laceration.

The employee was cutting a weld using a 4"" hand-held cutting wheel. The wheel broke apart and a piece of the wheel came off through the employee's glove and caused a laceration on his left hand.
