EE WAS LIFTING ONE END OF A 5 FOOT BEAM TO REMOVE A LIFTING CHAIN WHEN HE FELT A BURNING SENSATION AND A PULL IN LEFT SHOULDER. AGGRAVATION INCREASED OVERNIGHT AND REPORTED AT START OF NEXT SHIFT.

Employee was washing tail pulley of belt. He was positioned below the jaw crusher below and protective deck. A rock virbrated through an opening below the protective deck, falling and hitting the employee.

EE WAS ON A DOZER BACKING UP A STOCKPILE AND RANOVER A LARGE ROCK CAUSING THE DOZER TO RISE UP AND COMING DOWN ON TRACKS. HE HURT BACK FROM THE JARRING OF THE DOZER.

EMPLOYEE WAS CLEANING AT FEEDPAN WITH A BAR AND PULLED A MUSCLE IN HIS BACK WHEN HE WAS REINSTALLING THE GUARD.

EMPLOYEE WAS HIT ON THE HEAD WITH A ROCK (20-25 LB) THAT WAS SLUNG OFF THE CRUSHER SCREEN. HE RECEIVED A SLIGHT CONCUSSION & A LACERATION TO THE FOREHEAD THAT REQUIRED 10 STITCHES.
