Accident type, without injuries

A fall of Roof occurred int he old g-mains area. The afall occurred int he #5 entry between #17 bx thru #19 bk. Fall was 17 ft wide and approx. 200 ft long above anchorage. The 22 inch dischange line was damaged, no injuried or equipment involved. area supported with 6' belts and wooden posts.

Accident type, without injuries

A fall of roof occured int he #43 longwall panel teardown face. Fall was 14 ft wide x 16 ft highx 132 ft lontg. Area was supported with 16 ft cable bolts, 6 ft combination bolts dywag slings, t-5 channel and 30"" pumpable cribs. No equipment involved and no injuries. Cable bolts were strill in rod as support.

Accident type, without injuries

A fall of roof occurred in the #3 hoist track entry at #1 bk. Fall is 16' long to 18' long & 7' to 8' high across the entry. Noo equipment involved & no injuries. Area was supported with 6' resin bolts.

Over-exertion in lifting objects

PERSON WAS HELPING TO LOAD VULCANIZING TOOLS ONTO CAR TO SEND OUTSIDE. WHEN PERSON BENT OVER TO LEFT COOKER PLATES, HE FELT PAIN IN LOWER BACK.

EMPLOYEE WAS GRINDING KEY STOCK FOR CRUSHER BARS WHEN HE FELT SOMETHING HIT HIS RT. EYE UNDER HIS GLASSES.

EMPLOYEE WAS UNLOADING SUPPLIES FROM SCOOP BUCKET & STEPPED OUT OF THE BUCKET, TWISTING HIS RIGHT ANKLE CAUSING A SPRAIN.

Struck against stationary object

Injured was working on a high pressure water line. Water came on unexpected and threw him against rib causing bruises to left ribs.

Fall to the walkway or working surface

Person was helping to get belt discharge pulley through 6' door in stopping. The pulley moved unexpectedly and person lost balance and fell backwards. Person put his left land out to catch himself and this caused injury to left shoulder. Person is off for surgery to left shoulder as of 9/21/05.

Caught in, under or between a moving and a stationary object

Injured was carrying a post with another employee. The employee slipped forward causing post to go into rib. The injured had his finger between post and rib causing a laceration of the right index finger.

Struck by falling object

Person dropped crusher bit on top of his left foot while moving it out of his way so he could get bits for longwall shear drum.

Injured was holding a 1"" water line. Another man was hitting the coupler with a hammer when a piece of metal came off and went into his right forearm.

WAS RUNNING CHAIN HOIST WHEN HOIST FELL HITTING EMPLOYEE IN BACK OF SHOULDER AREA.

Injured was carrying a door with another employee. He lost his footing and slipped in mud and twisted back.

Injured was threading 1"" steel cable into a thimble. Cable slipped out of thimble and struck him on hard hat right side causing a slight concussion.

Fall onto or against objects

EE was installing light ballast for support during longwall set-up. EE was moving from #22 to #23 support and lost balance. He grabbed hose that was not attached and fell forward, striking his right shoulder against side flap. EE went to doctor on 4-15-05 & got cortisone shot. SURGERY REQUIRED - WILL BEGIN TO MISS WORK AS OF 8/25/05.

Fall to the walkway or working surface

EE WAS WALKING ALONG TRACK TO GET TO MANTRIP AT END OF SHIFT. EE STEPPED IN A HOLE BESIDE THE RAIL AND LOST HIS BALANCE. EE FELL FORWARD, CATCHING HIMSELF WITH HIS LEFT ARM, CASING PAIN IN SHOULDER. EE WENT TO DOCTOR ON 3/28/05 AND IS NOW OFF WORK FOR SURGERY TO SHOULDER.

Struck by falling object

Person was cutting wire screen down and scaling loose roof rock down at crosscut into #2 entry, that had just been mined through. A piece of rock fell on his left foot while he was scaling it down from roof, causing injury to left foot.

EE WAS JACKING UP A RAIL WHEN JACK SLIPPED AND RAIL STRUCK EE ON RIGHT ANKLE CAUSING A BRUISE.

EE WAS ON LONGWALL WHEN A PIECE OF ROCK FELL FROM BETWEEN ROOF SUPPORTS STRIKING EE ON HEAD AND RIGHT FOOT CAUSING A BRUISE

Person was walking to piece of equipment and stepped on a rock and twisted ankle and fell.

Injured was shoveling spillage along belt line when a piece of rib rolled striking him on the right wrist causing a bruise.

Employee was walking through a 4'X 6' mandoor at the submains belt starter (3 break). He slipped and fell causing injury to his ankle.

Injured was rockdusting, he stepped on a piece of belt that was covered with mud, slipped and fell causing a fracture of ribs left side.

Caught in, under or between a moving and a stationary object

INJURED PERSON WAS SETTING LATCH MECHANISM ON 5 TON RAIL JACK BY HAND AND WHEN ANOTHER PERSON OPERATED THE JACK, HIS RING FINGER WAS CAUGHT IN THE MECHANISM CAUSING INJURY TO FINGER.

Fall from ladders

EE SAID EE WAS PULLING SANDBLASTING HOSE UP LADDER WHEN EE TOOK PAIN IN BACK AND SLIPPED OFF LADDER LEAVING EE HANGING FROM SAFETY HARNESS APPROX 2 FT OFF FLOOR

Over-exertion in lifting objects

EE WAS HELPING TO HANG MONORAIL AND FELT PAIN IN LEFT SHOULDER CAUSING A PINCHED NERVE

PERSON WAS HELPING TO INSTALL 1 1/2"" TEE IN EMULSION LINE FOR LONGWALL RETURN LINE. ANOTHER PERSON WAS GOING TO DRIVE THE TEE IN PLACE AND STRUCK INJURED PERSON'S MIDDLE FINGER WITH SLEDGE HAMMER CAUSING FRACTURE AND LACERATION TO FINGER ON RIGHT HAND.

PERSON WAS WALKING THROUGH MUD AND WATER TO CHECK PUMP IN RETURN. PERSON STEPPED ON SOMETHING BURIED IN THE MUD AND TWISTED HIS LEFT KNEE. PERSON WENT TO DOCTOR ON 12/29/05 AND GOT CORTISONE SHOT IN LEFT KNEE. PERSON REPORTED TREATMENT TO COMPANY ON 01/02/06.

PERSON WAS ATTACHING A ""CIU"" BOX TO A LONGWALL SHEILD MOUNTING BRACKET. PERSON PUT ONE BOLT IN AND LET GO OF THE BOX TO PUT ANOTHER BOLT IN TO HOLD IT. THE BOX FELL WHEN PERSON RELEASED IT AND IT STRUCK EE IN MOUTH, LACERATION TO UPPER LIP , 7 SUTURES TO CLOSE

PERSON WAS WALKING UP STEPS AT OVERCAST ALONG BELTLINE WHEN HIS FOOT SLIPPED OUT AND HE FELL AGAINST THE HAND RAIL STRIKING HIS RIGHT ELBOW. SWELLING IN ELBOW HAD FLUID REMOVED FROM ELBOW.

Absorption of radiations, caustics, toxic and noxious substances

PERSON WAS ROCK DUSTING ALONG THE BELTLINE AND GOT ROCK DUST IN LEFT EYE. PERSON HAD EYE FLUSHED OUT AT EMERGENCY ROOM AND GOT RX FOR EYE.

Struck by flying object

INJURED WAS USING ""RED DEVIL"" ROPE COME-ALONG TO HELP POSITION TAILGATE CUTTER MOTOR ON LONGWALL SHEAR. THE HANDLE SLIPPED OU OF THE COME-ALONG AND STRUCK PERSON IN MOUTH CAUSING LACERATION TO LOWER LIP WHICH REQUIRED SUTURES TO CLOSE

Over-exertion in pulling or pushing objects

While inserting 6 ft roof bolt into hole in roof, person felt a sharp burning pain in lower right abdomen. Person was standing on left side platform reaching out to install bolt.

Struck by falling object

EE WAS INSTALLING ROOF BOLTS IN TEARDOWN ROOM FOR LONGWALL PANEL. EE WAS PUSHING BOLT UP INTO HOLE WHEN THE EXTENSION WRENCH BROKE AND THE WRENCH CAME BACK AND STRUCK EE ON THE FOREHEAD, CAUSING LACERATION.

Person was unloading 60"" top belt structure from back of pickup truck. When he lowered the piece of structure the side idler roller rotated and caught his little finger on right hand between center roller and side roller causing laceration.

Person was using rail jack to lift flat car back onto track after de-rail. Person was using slate bar to operate jack to raise car. The jack did not latch on the down stroke and slate bar flew back up and struck person on left side of face.
