Over-exertion in lifting objects

EE was lifting a sieve up from inside the screen to spin it because of wear when EE heard a pop and felt pain in EE's right elbow.

Over-exertion in lifting objects

Employee was lifting a 3 ton chain hoist (Roughneck Brand from northern tool + equipment -weighs 48 lbs.) out of the hole we were working when employee felt something pull on the left side of stomach area.

Fall from ladders

The employee was descending down a metal permanent ladder backwards and slipped on the second rung from the bottom landing on EE's feet twisting EE's right knee in the process.

Removing bad 4"" steel channel from the side of the thermal dryer to install new siding to be installed. EE was holding the opposite end that was being cut free when it was cut loose it seesawed and smashed EE's right hand thumb up against another piece of metal.

Struck against stationary object

Hit head on an X-brace beam with hard hat causing neck pain.

Employee was doing preparation plant maintenance repair and utilizing a hammer while extracting a retainer pin when EE miss-struck the pin with the hammer, and incurred a puncture wound injury to the top of left hand.

Employee was taking well water samples using a 1"" bailer. When the bailer exited the well casing it stuck the employee in forehead causing a laceration that required medical treatment, (2) sutures.

Cutting the tape off of the cord that was keeping the cord rolled up to use it and when EE went to pull the tape off after it had been cut EE forgot to close the folding razor knife and stabbed self in the chest.
