Accident type, without injuries

A fall of roof occurred at the #6 BK intersection off the E-1 track entery just outby #15 seal. Fall was in the intersection and was approx. 16' X 16' X 6' high. Area was cleaned up and supported.

Caught in, under or between a moving and a stationary object

The Jeffrey Locomotive EE was operating struck a longwall support that was loaded on a low boy parked at 66 brk switch on E-1 track. Locomotives were being staged at this location for transport to Submains. Locomotives were advancing one at a time.

Over-exertion (Not Elsewhere Classified)

Person was working on changing a hydraulic valve boom. While tightening a fitting, the ladder he was standing on shifted and he grabbed onto a pipe pulling his right shoulder. Person now has to have surgery on rotator cuff and began to miss time on 5/2/07.

EE was walking up grade from propane tank to company truck. As he was walking up hill he felt a sharp pain in right ankle and leg. EE went to chiropractor on 5/15/06. *Person had to have surgery on right ankle and began to miss time on 2/14/07.*

EE was helping move steel beam, which struck him in the left knee.

Fall to the walkway or working surface

Person was carrying bag of rock dust on shoulder to use for dusting on beltline. When he walked past starter box he stepped on rubber mat and slipped and fell causing injury to left knee. Person now has to have surgery on knee, first day off is 9/21/06.

Contact with hot objects or substances

Person was using a cutting torch to remove bolts in air-lock door. Person was using left hand to hold cables up out of the way and cutting bolts with right hand. He slipped and lost balance and the torch in right hand came in contact with left hand causing burn to palm & thumb.

Fall onto or against objects

Person was walking down main belts on fireboss exam. At #2-#3 bk on #1 belt he slipped and tried to catch himself. His left hand caught top belt roller structure and his ring finger on left hand caught between roller and frame causing injury to ring finger.

Fall to the walkway or working surface

Person was walking down a bank that was slick and lost his footing. He fell and landed on his right arm causing injury. Person has gone to chiropractor.

Struck against stationary object

Person was working on 3rd floor of plant on vibrator for Wedag changing ""V"" belts. Person tried to go to other vibrator and ran head into valve for waterline causing him to jam neck. Person went to chiropractor.

Injured person was helping to change out ram jack on longwall support #30. Person was using come-along to pull jack upward to remove pin, when the jack came loose and struck person in forehead causing laceration. Required 6 sutures to close the laceration.

Person was cleaning inside off at chute to do some welding. While cleaning, some dirt and rust got into his right eye. Person went to ER after work and they removed a small piece of metal.

Struck by falling object

Person was unloading post from back of hummer on the longwall section. The post end came down and caught his ring finger between post and monorail dolly which was laying on the ground. The distal end of right ring finger was fractured.

Injured person was helping to load 20"" ventilation tubing into scoop bucket. Person got his left hand caught between end of tubing and side of scoop bucket, causing injury to left little finger.

Struck by flying object

Employee was grinding metal on bench grinder when a piece of metal went behind safety glasses hitting employee in left eye.

Person was setting additional support for roof. Person had a crib block on top of water prop and was raising prop towards roof. When crib block was being pressuried against the roof it kicked out and fell striking person on right hand causing injury to ring and middle fingers.

Person was operating forklift in warehouse, he had stopped and was getting off. When he stepped down on to the floor he felt a sharp pain in the lower left side of his back. Accident was not reported until feb 14, 2006. Person was treated by a chiropractor.

Person was using utility jack to lower section of belt take-up structure into place. The structure shifted and the jack kicked out. When this happened, the jack handle struck person on right side of face. Sutures required on inside of right cheek.
