Over-exertion in lifting objects

I am filing this report because employee called me on the 18th of December stating he had to have emergency surgery on the 17th of December stating that he thought he had come down with the flu on the 8th of December and had to take off work until the 11th of Dec & returned back to work still feeling ill & worked 4 days until they found a hernia in his stomach.

Over-exertion in lifting objects

Employee pulled muscle in upper right arm while picking up material to move. Employee did not start missing time for this accident until after being examined by a physician for arm pain and it was determined the pain was the result of November 6, 2013 accident. First day missed January 13, 2015

Struck by falling object

Employee was sitting metal plates 3/8 x 48 1/2 x 16 inches against pallet of flock when plate fell over hitting him left leg.

Over-exertion in lifting objects

Employee was setting up scaffolding-while moving scaffolding employee felt a sharp pain in back & going down left leg.

Struck by... (Not Elsewhere Classified)

Employee was struck on right cheek with compress air while performing maintenance work. The employee was examined and released by a medical provider at the time of the accident with no reportable treatment. The employee has since received medical treatment (laser surgery) to removed embedded coal particles from his right cheek making this accident reportable.
