Flash burns (electric)

Immediate notification was made when information was received that an incident occurred at the thermal dryer. There was an arc inside the electrical cabinet which resulted in a fire and injury. At the time this information was received the severity of the injury and the fire were unknown. The fire was immediately extinguished and the injury was not life threatening.

Fall from ladders

As employee was investigating a leaking pipe, the step ladder he was using kicked out from beneath him. Ladder was positioned on stable ground - all four legs. Employee fell from the fouth step, glancing off underflow box and onto floor.

Fall down stairs

EE was approaching steps at the dryer and lost his balance and tumbled down the steps hurting his left hand.

Over-exertion (Not Elsewhere Classified)

Employee slipped on ice covered roadway while working outside at the loading facility. Employee continued to work until May 16, 2011 when his doctor took him off awaiting knee surgery.

Employee was unloading a 5 gallon can of grease from the back of a pickup truck. When he slid it off the tailgate the can dropped slightly causing a jerking action straining the employees back.

Struck by flying object

EE was brushing dirt off of a truck frame when strong wind blew dirt up under his safety glasses getting dirt in his right eye. EE went to the hospital and got his eye cleaned out.

Employee was chipping rust off of steel with hammer when piece flew under safety into left eye. Accident was not reported until 2/24/11.
