Struck by... (Not Elsewhere Classified)

EE was installing coal mill ductwork above the building roof line. The ee welded the duct to hold it into place in order to fit all the duct together. The foreman called for the duct to be released from the crane & at that time, the duct broke lose pinning ee between duct & roof beam. The ee was instructed on the installation of the ductwork.

Fall from machine

Employee was pulling on a bar and fell backwards, spraining his left ankle.** original EDoc # 288402**

Struck by powered moving object

Employee was loading tailings into a dump truck. He didn't think the screw was working so he backed up to get a better view and was struck by the corner of a railcar. Employee stated he heard the locomotive horn but didn't realize how close he was to the tracks.

Contact with hot objects or substances

Employee was cooling down hot bearing, opened valve for coolant line. Hot liquid sprayed out, splashed off a heat shield and struck employee causing burns on hands, arms, back, neck and face. Also struck was an employee of Mississippi Lime. Valve was then closed and more air and water applied to cool it.

Struck by rolling or sliding object

EE was relocating a slide valve for a coal mill when the valve became wedged on a bolt between pipe. As the EE was trying to free the valve, a fellow employee shook the pipe. The valve came loose and rolled trapping EE's left hand between the valve and pipe. EE was transported to a local treatment facility for treatment. He was treated for a bruise and laceration to left hand.

Fall to the walkway or working surface

Employee was off shift and walking to the parking lot and fell at his car. Employee stated he forgot his keys and was in a hurry.

Contact with hot objects or substances

EE was welding picking eyes on a rock box, when a piece of hot slag fell onto left glove, burning through glove and onto wedding band on left ring finger. 2nd degree burn around finger where wedding band was worn.

Struck against stationary object

While using an allen wrench to tighten a bolt it slipped off and he hit his right forearm on a gear causing a laceration. The laceration required sutures.

Contact with hot objects or substances

While assisting an FFE engineer, the engineer opened a line with heated glycol which sprayed on his back. Employee received second degree burns on his back.

Employee stated while servicing a cylinder he attempted to take it apart and it suddenly came open striking him above his safety glasses.

Struck against stationary object

While unloading pallets from a dump truck, the employee cut his finger on a pallet. The cut required a couple of sutures.**original EDoc # 300870**

Over-exertion (Not Elsewhere Classified)

While hand scaling with a bar employee stated he felt a pull and sudden pain in his left shoulder.

Struck against stationary object

While closing a bottom slide on a railcar the bar slipped out of the caption which caused his hand to hit the side of the railcar. He received a minor fracture to his right little finger. *Addendum: X-ray later revealed there was no fracture.
