Struck by... (Not Elsewhere Classified)

Employee stated he was scaling and sat the scaling bar on top of his foot. When they moved the cage the bar contacted the roof and pushed the bar onto his toe. His right middle toe was cut near the joint. He is currently missing time from work.

While cleaning out concrete footing with a backhoe the operator shifted the bucket to the left and boomed down. The edge of the bucket swung farther than expected and a tooth on the bucket struck the left foot of an employee that was directing the operator.

Over-exertion (Not Elsewhere Classified)

Employee stated he was coming down the steps of a front end loader and slipped catching himself with his left hand and straining his shoulder. He had orthoscopy surgery to his left shoulder and began missing.

Fall onto or against objects

While walking out of the lab on the firing floor employee stated he missed a step and fell against a wooden table. He was diagnosed with a rotary cuff tear and had surgery on 06/02/2010. June 2, 2010 was his first day missed and worked full duty until surgery.

Fall from machine

Employee stated he was attempting to release a manual brake on a railcar and slipped falling to the ground and catching himself with his right arm and overextending it. He was initially diagnosed with a strain but a small tear was later found. He underwent a surgical repair and began missing time on 08/04/2010.

Struck against a moving object

EE was operating a fork truck and crossing a set of railroad tracks when the forks or load caught a rail causing the fork truck to stop abruptly. The employee was caught by his seatbelt but he bent forward and hit his chest on the steering wheel.

Struck against stationary object

While shovelling by a belt the employee stated he hit his left pinky finger on the handrail to the stairs and fractured his little finger. The employee was sent to a surgeon and the finger was pinned. He is currently on light duty.

Over-exertion in lifting objects

Employee stated while opening a railcar lid he felt pain in his lower back. He was sent to a doctor where he was given light duty.

While stepping up on a bumper of a pickup employee stated he felt pain in his right knee. The physician instructed him to work light duty.

Absorption of radiations, caustics, toxic and noxious substances

Lime dust blew up under safety glasses when employee leaned down over cooler area, affecting right eye.

Employee stated while cutting a cable with a knife it slipped and cut the palm of his left hand.

Fall to the walkway or working surface

Employee stated he tripped on the edge of a manhole cover and fell forward, landing on his knees and hands. He had ""slight"" discomfort in his shoulder. He did not request medical until later at which time he was diagnosed with a tear and required minor surgery on 09/17/2010. He is an operator in a booth and is not on light duty and did not miss any time.

Struck by flying object

While cutting or grinding the employee stated he got a small sliver of metal in his left eye. The employee was sent to the eye doctor and the sliver was removed and a prescription medication was given to him. The employee further stated he was using goggles but a piece got behind them.

Absorption of radiations, caustics, toxic and noxious substances

Employee went to check the level in a tank he did not see a broken bag of biocide on top of the tank. When he bent over and opened the tank inspection hole the pressure from inside the tank blew upward blowing biocide into his face. The employee did not go to the doctor until much later and was referred to a specialist on 03/04/2010 and given a prescription medication.

Employee stated after operating a mechanical scaler over several years her wrists became painful at night. She was advised to have surgery and began missing time on 10/13/10.

Employee stated his fingers became numb after repetitive work such as using a sledge hammer. He underwent surgery for carpal tunnel on 09/17/2010 and is currently missing time from work.

Employee stated after years of scaling his wrists became painful at night. He was seen by a physician and diagnosed with bi-lateral carpal tunnel. He received surgery on 05/19/2010 and is currently missing time. (Occupational illness)
