Struck by falling object

Employee was moving the stone tripper when a steel bean fell hitting him on the upper back/shoulders.

While cleaning out a chute with a scaling bar a large amount of debris released striking his right ankle.

Over-exertion in lifting objects

Employee stated he picked up a drill steel weighing approximately 90 by himself when he felt pain in his lower back.

Fall from machine

While climbing up the ladder of a front end loader he fell approximately 3-4 feet on his right hand. The hand is fractured and he has been placed on light duty.

Struck by falling object

Employee stated he was using a Max Vac system when the hose became hooked on something. As the hose sprung forward it was covered with lime and flew lime into his right eye. Employee was given a prescription for his eye and instructed to work light duty.

Caught in, under or between a moving and a stationary object

While unchoking a hopper with an air lance the employee stated he smashed his left ring finger between the air lance and the metal lip on top of the hopper. The employee received 3 sutures to his finger.

Absorption of radiations, caustics, toxic and noxious substances

Employee stated while using a pneumatic chisel for material removal a piece of hydrate went under his glasses and into his eye. He went for treatment where he was given a prescription medication for his eye.

Struck against stationary object

Employee stated he was un-choking a feeder with a hammer and struck a protrusion with his hand as he was swinging. He received 7 sutures and went back to full duty.

Pain and numbness in left elbow and little finger and ring finger that has been getting worse from repeated use. Employee started missing work on 6/1/2015.

Employee stated while he was removing some bolts from a motor his right elbow became painful. He has been having problems with the elbow over a long period of time doing repetitive motion duties. He was diagnosed with an occupational illness on his right elbow and having surgery.

While cleaning scrubbers with a sledge hammer for years his wrists became sore. He first went to the doctor in September and began missing time from work on 09/28/2012 for surgery (carpal tunnel).

Employee stated that he was having pain in his right hand and thought it might be carpal tunnel. When asked what caused it he said it was from shoveling a long time ago.

Employee stated that numbness in his hands began after years of using air chisels, needle guns and air wrenches. He was diagnosed with bi-lateral carpal tunnel and given surgery to correct his condition.

Employee stated that his hands are causing him pain at night. He has been a jumbo drill operator for years using both a joy stick and hydraulic controls to repetitively operate the machine. He has been diagnosed with carpal tunnel and is off work starting 02/22/2012.
