Over-exertion in pulling or pushing objects

Employee stated while throwing a rail switch EE felt a ""pop"" in left knee. The employee was sent to a doctor, then a specialist and is scheduled for surgery.

Employee stated was pushing a hopper under a scrubber when employee felt a ""pop"" and developed a bulge in mid section. Employee was sent to a physician where employee was diagnosed with a hernia. Employee has been sent to a surgeon to schedule the repair for the hernia

Flash burns (electric)

An electrician was injured while trouble shooting a controller bucket rotary disconnect for #1 dust collector fan. An arc flash resulted causing burns to employee. This is reportable because it is a lost time accident.

Employee apparently walked in the spray of a high pressure sprayer. EE received a laceration on right upper leg requiring surgery. Employee is currently off work.

Struck against stationary object

Employee stated they were walking across the shop floor when they slipped on a piece of wood. They began missing on 06/24/2016 because they were placed on light duty and will receive surgery on left knee on 06/28/2016.

Absorption of radiations, caustics, toxic and noxious substances

Employee stated while turning a wheel to move a slide and release a choke up of material lime fell from above and landed between EE's glasses and into EE's right eye. The eye doctor advised EE stay off work until healed.

Contact with hot objects or substances

Employee stated EE was standing behind an employee that was using a cutting torch and a piece of hot slag fell into EE's right eye. EE was taken to the eye doctor where the piece of slag was removed. Employee missed 3 days from work.

Fall from ladders

Employee stated while climbing down a ladder and slipped causing them to miss the bottom step. Employee further stated they overextended their right knee and twisted left knee. Employee went to their personal doctor for treatment. On 01/18/2017 employee contacted me stating it was work related and requested treatment. Employee is currently missing time.

Over-exertion in pulling or pushing objects

Employee stated EE was pulling bags out of a bag house when EE felt tightness & pain in EE's groin. Employee was sent to a physician where EE was diagnosed with a hernia. Employee is scheduled for surgery on 10/18/2016.

Fall from machine

Employee was assisting in opening the tarp on a truck to load by standing on the fuel tank. The tank was covered in ice causing the employee to slip and fall to the ground landing on both feet but spraining the right knee upon landing.

Over-exertion in wielding or throwing objects

Employee stated that while cleaning #9 south scrubber with a sledge hammer ee struck left elbow on a beam support. Employee was given light duty by a physician.

Employee stated that while ee was walking up a stairs ee left knee became painful and swollen. EE was taken to a doctor where ee was given light duty.

Employee stated while working on the Peerless Stedman Mill EE was pulling on a ratchet and felt severe pain in EE's upper back. EE is currently working light duty.

Caught in, under or between running or meshing objects

Employee stated that safety glasses fell off face and when employee bent over to pick them up employee laid hand on steel structure of the pan conveyor. A steel roller ran over the tops of 4 fingers and fractured one of them.

Employee stated that while cleaning scrubbers at MRH2 employee received lime burns on both wrists. The doctor ranked the lime burns as 2nd degree and placed ee on light duty.

Employee was dismounting the locomotive and twisted left ankle.

Fall to the walkway or working surface

Employee stated while stacking bags of R-2 stone EE was standing on a pallet and a board on the pallet broke causing EE to fall forward. Employee was taken to the clinic and was diagnosed with a right knee strain and given light duty.

Employee stated that while EE was releasing a brake on a railcar with a brake stick EE stepped into a hole, twisting their right knee. EE was taken to the emergency room where EE was given a splint and light duty.

Employee was cleaning on a steep and uneven roof and turned left ankle. Employee did not request medical until 04/26/2016. EE was placed on light duty and given a splint.

Employee stated employee was opening a valve when employee felt a pop. Employee was seen by a physician. The initial x-ray report was negative. On January 10th the employee was contacted by the doctor stating the x-ray show a fractured rib.

Employee stated while ee was taking off goggles lime fell off the goggles into eyes. EE was treated by a local eye doctor and a prescription medication was used in eyes.

EE was replacing a rotary feeder valve. As EE was drifting the valve out the valve shifted and mashed right middle finger causing a cut that required stitches.

Struck by falling object

Employee stated that EE was climbing out of RK2 Halo Duct while holding onto the manhole door. EE stepped into a hole and dropped the door onto EE's left thumb causing a compound fracture and severe laceration. Employee was taken to a surgeon where the thumb was repaired. Employee is an Engineering Project Manager and will continue working full duty.

Employee stated that while standing outside the Specialty office a piece of hydrated lime blew into EE's eye. The employee was seen by an optomologist and was treated with a prescription.

Employee stated as EE was stepping out of #6 kiln EE caught EE's armpit on the bracket of the burner pipe. EE received 14 sutures to EE's left armpit.

Struck by falling object

Employee stated while working on the RK2 preheater ram EE was removing a 2"" inch steel pin when it dropped onto right little finger. EE further stated it both smashed and cut finger. EE was taken to the hospital for treatment and the ER physician closed the wound with 2 sutures.

The employee was working and burnt upper inside arm area with hydrate dust, resulting in a second degree burn.

Employee stated that EE was changing a motor on the east lime screen and received lime burns on both forearms. Employee went for a follow up treatment on 07/07/2016 and a ""new"" doctor in the occupational medicine department re-classified the burns as 2nd degree. (Note-medical notes indicate re-classification of burns).

Employee stated while repeated manual work of pulling the hose out on the fuel truck, they began to feel pain in the left shoulder. Employee had surgery on 4-26-2018 on left shoulder to correct the cubital tunnel syndrome.

On 5-9-16, employee reported of numbness in both hands. Employee was diagnosed with carpal tunnel. Employee is having surgery to correct this illness on 7-29-16 and began missing work on this date.
