Accident type, without injuries

The super sucker was being used to clean the scale pit at the Mississippi Rotary. The engine quit. When the operators came out of the pit they found the engine compartment of the truck on fire. The fire was extinguished in approximately 29 - 32 minutes.

Employee stated he was using the supersucker to vacuum to unload a railcar when the 4"" inch plastic hose jumped and knocked hydrate into his left eye. Employee received a small burn to his left eye and was placed off work and given a prescription medication.

Over-exertion (Not Elsewhere Classified)

Employee stated, while working on a PQL mill breakdown, he injured his shoulder. Employee received shoulder surgery on 01/25/2016 and returned to light duty on 01/29/2015.

Employee stated that he was replacing a screen when it became jammed in the slot he reached down with his hand and it slid backwards pinching his little finger on his right hand. He fractured the tuft of the little finger and tore his nail off.

Employee was walking on the firing floor looking for a gas blind. As he was walking his head hit a cable and lime dust fell down behind his safety glasses into his Right eye.

Employee was opening a door on a dust collector duct door when hydrate blew out of the duct into his face and eyes.

Employee was changing a bearing on a conveyor tail pulley. During repair employee received a metal sliver in the palm of his left hand. Medical treatment was required to remove the piece. Employee returned to work to complete the remainder of his regular scheduled shift with no work restrictions. This is a reportable accident because there was one suture.

Employee stated he was opening a gate with a chain and dust fell into his eyes. He had safety glasses on. He was given a prescription by a doctor.

Over-exertion (Not Elsewhere Classified)

Employee stated while using vibrating tools over the years his wrists, elbows and thumbs became painful and numb. This was diagnosed as bi-lateral carpal tunnel and surgery was done on 09-04-2015.
