Struck by... (Not Elsewhere Classified)

EE and other co-workers were trying to connect a 42-in. inlet duct to a top reversal box through an opening in the building. The EE was inside the building between the inlet duct and reversal box. When the inlet duct unexpectedly came through the opening, it struck the injured employee in the head causing severe head injury resulting in death on 6/19/2007.

Fall from ladders

Employee was working on a hydraulic line (non pressured) when fluid came pouring out on him. He was on a ladder and when the fluid came out he climbed down the ladder and fell the last 3 rungs.

Over-exertion in pulling or pushing objects

Employee was raking coal spalls out of #7 kiln hood when he felt pain in his shoulder. He did not beging missing time until 01/09/2008 when he had shoulder surgery.

Fall to the walkway or working surface

While walking to plug in a block heater on the tractor he fell on some ice. He did not complain until recently when he was sent to a doctor and a problem was found. He began missing on 04/29/2008 for surgery.

Contact with hot objects or substances

Employee stated he was cleaning scrubbers when the recirculating pipe came apart and he received second degree burns to his arm and chest.

Over-exertion in wielding or throwing objects

While employee was throwing a hose, he felt pain in his low back.

Struck by... (Not Elsewhere Classified)

Employee stated he was looking for a hydraulic leak when he lacerated his finger on his right hand. The laceration required a minor surgical procedure to repair.

Struck against stationary object

While cutting a bolt with a hand grinder his hand slipped into the wheel and he received three sutures to his left middle finger. He was wearing gloves.

Caught in, under or between a moving and a stationary object

While lifting a tail pulley into place he caught his finger in between the pulley and a piece of steel. The end of his right ring finger was fractured and received 4 sutures.

Struck against stationary object

Employee was walking and stepped into a hole. He caught his leg onto a piece of grating and cut his leg which required sutures. He did not miss any time from work and was not placed on light duty work.

Employee stated while bagging stone product his wrist became sore. He was diagnosed with carpal tunnel and had surgery on 01/30/2008.

Over-exertion (Not Elsewhere Classified)

EE stated he has been using hand & vibrating tools for years causing numbness in both wrists. He did not miss work until 4/25/07.
