Accident type, without injuries

NO INJURY -- While making weekly exam, a rock fall was discovered in #2 x-cut, #7 entry on 2nd left room in return. Air movement or travel way was not affected. The fall was 1"" to 4"" thick, 18' wide, and 50' long.

Caught in, under or between a moving and a stationary object

A Mack DM-600 tandem axle lube truck was descending a grade into the active work area of the mine when the operator lost control of the truck and either jumped or was thrown from the vehicle. The truck struck an embankment and overturned on its left side resulting in fatal crushing injuries to the victim. He was pronounced dead at the scene by the coroner.

Struck by falling object

Ee was standing beside the rib, when a piece of rock fell from the roof and hit him in the head and shoulder. Injuries are numerous but not life threating or debilitating. Doctors are saying he will be off for two or three months, maybe more.

Over-exertion (Not Elsewhere Classified)

Ee was bending a 4' bolt in low top. Caused him to have a hernia. He is now off due to surgery.

Over-exertion in lifting objects

Miner was carrying rock to put on the bottm of a curtain and pulled a muscle in his groin. Ended up very minor and he missed one day and came back to work.

Struck by falling object

Ee is a repairman. He was using a buggy for transportation when he hit a timber with the buggy. This moved the timber so that a piece of rock fell from the top and hit him on the neck and shoulder.

Struck by falling object

Miner is a miner operator/miner operator helper. A small piece of draw rock fell from the top and him him on the lower left leg and foot. No bones were broken. He was off three days and came back to work no problems.

Fall onto or against objects

Ee was hanging cable (for tracking) in the intake air course when he fell hitting his back against the man trip. He worked two days before he decided to go to the doctor. Doctor said he had cracked his ribs and should be better in a few days.

Struck by... (Not Elsewhere Classified)

While drilling a hole for a roof bolt, the drill steel hit EE in mouth. This cut his lip and chipped his teeth. Stitches fixed his lip and a dentist took care of his teeth. Missed one day of work. Back to full duty with no problems.
