Accident type, without injuries

A non-injury roof fall was discovered in the intake off of #16 belt at spad number 1491.5. Neither ventilation or primary escapeway lifeline were effected due to multiple entries of travel. The fall is between pillars, not in an intersection, and in a well entry. It is approx. 4 1/2 ft thick , 25 ft length, 16ft width. The effected area is to be cribbed off.

Accident type, without injuries

A non injury roof fall was discovered at break #10 on the #6 belt in the #1 entry. It is app. 75ft long and 10ft thick and 19ft wide. The effected area is to be cribbed off or timbers. While doing a weekly exam.

While backing up roof bolter, cable reel struck employee in the right elbow. Resulted in a pulled muscle and chipped elbow.

Over-exertion in lifting objects

Employee pulled lower back and groin muscle while lifting crib block.

Employee was shoveling the 001 section tailpiece and hurt his lower back.

Employee's leg was ran over by a buggy and a welding cart while holding the door open, resulting in bruising and scraping.

Contract Employee slipped and fell while working on #2 head drive. He twisted his right knee and hit it against the turn buckle.

Employee was riding mantrip when he was caught between the mine beams and the mantrip. He pulled a muscle in his lower back.

Employee was shocked while moving car cable out of the roadway. He was wearing rubber gloves.

Employee caught left hand in rib fence wire while installing fence on the coal rib. Twisted left wrist.

Employee pulled groin area while picking up belt structure.

Employee dropped a ""come-a-long"" on his left foot while splicing a belt.

Employee cut his right leg above knee while cutting a piece of belt., knife slipped and made a 2"" incision.

Contact with electrical current

Employee was holding a shuttle car seat while the repairman was welding a bracket on. The employee was shocked when the welding rod touched the frame. Note: The power was disconnected from the shuttle car at the time of accident.

Employee was pulling a pump cable. The cable hit him in the left eye causing mud and coal to get beneath his eye lid. Eye was irritated and swollen.

Struck by rolling or sliding object

Contract employee (Bates Contracting) was injured while servicing the continuous miner. A piece of rib measuring 10 ft long x 40 in. tall x 20 in thick rolled from the top of rib striking employee from behind and pinning him against the continuous miner.
