Accident type, without injuries

Top and had been supported with 48"" glue bolts. Roof fall is approx. 14' thick, 60' long, 18' wide. Fall occurred in the intake air cause, between brks 2-3 of the #11 belt line. Ventilation checked inby the fall area, no change in ventilation. Note: mine has been in non-producing status since April 1, 2016. There is no active section at time of fall.

Over-exertion in lifting objects

Employee was cleaning the shop at mine location. As employee was dumping a wheelbarrow full of dirt/mud/garbage into a front end loader bucket, felt a sharp burning and stinging sensation in lower back. Employee did not feel the need for medical/first aid. Worked the next three shifts and went to dr on 12/8/16.
