Accident type, without injuries

A non injury roof fall was discovered in the #1 south panel in the #6 entry at 2375 & 2386 spad App 40ft long 18ft wide and 10ft thick. Neither ventilation or primary escapeway were effected. The affected area has been cribbed off and dangered off.

Over-exertion in pulling or pushing objects

Employee was loading bolt supplies on to bolt machine. Employee pulled roof bolts from bundle and twisted. Employee said his upper back popped between his shoulder blades.

Over-exertion in pulling or pushing objects

Employee pulled/strained lower abdomen area while lifting/pulling safe haven.

Fall onto or against objects

Employee walked to the back of the walk through roof bolter to get a box of glue. While carrying the box of glue back to the head of the roof bolter, he fell in the walk way of the bolter.

Struck by rolling or sliding object

Employee was helping other rib bolt machine operator get rib fencing in place to bolt the rib, when rib rolled out (coal and rock) striking the employee on left arm and body.

Caught in, under or between a moving and a stationary object

Contract employee (ID #5UF) smashed his right hand while exiting the scoop. He caught his hand between the scoop door and the canopy.

Struck by... (Not Elsewhere Classified)

Employee was splicing a piece of conveyor belt when his hammer chipped. A piece of metal went into his right hand between his index finger and thumb.

Fall to the walkway or working surface

Employee slipped/fell at #2 head drive while pre-shifting. He bumped his knee and head on the ground.

Fall onto or against objects

Employee slipped and fell into miner heads while setting bits.

Struck against a moving object

Contract employee (Contractor ID #5UF) was riding the mantrip when it hit hole, jarring his lower back.

Struck against stationary object

Employee was struck by the scoop deck door while exiting the equipment. The door struck the employee in the back of the head and neck.

Over-exertion in pulling or pushing objects

Employee pulled/strained right shoulder while cleaning out a rib bolt.

Over-exertion in lifting objects

Contract employee strained/pulled back muscles while carrying a planatary to the section power center.

Struck by falling object

Employee was putting up a bolt with a strap when a kettle bottom fell out of the roof striking employee and knocking him down. Resulted in a broke right leg above the ankle.

Over-exertion in pulling or pushing objects

Employee pulled/strained lower back while pulling a ""come-a-long"" to make a belt splice.

Struck by falling object

A piece of rock fell from the roof striking/pinning employee on the lower half of his right leg while he was working on the feeder. Resulted in a gash and broken leg.

Struck by falling object

Employee raised up from the kneeled position to get a bar. A hammer was on the bar and fell striking him above the right eye.

Fall onto or against objects

Employee was walking in the #1 entry, carrying a torque shaft to the continuous miner when he fell on uneven bottom striking the tail on the miner with his mouth.

Struck by falling object

Man door fell on employee's hand while scooping at #3 head drive. Hand was swollen and bruised.
