Accident type, without injuries

Idle Mine A non injury roof fall was discovered at #6 break on #6 belt in the #2 entry in the intake. The affected area is to be cribbed off. The fall was approx. 20' x 24' and about 9' thick.

Caught in, under or between a moving and a stationary object

Employee was roof bolting when he caught his left thumb between the mine top and the drill chuck on the roof bolter.

Over-exertion in lifting objects

Employee strained and pulled right arm tricep while shoveling belt line.

Struck by flying object

A piece of debris in employees eye. Another miner was hammering and a piece of debris flew in employees eye.

Struck by rolling or sliding object

Rib rolled and hit employee. Employee complained of head, neck, and shoulder pain.
