Employee was crossing over a belt crossover, slipped and fell. EE's left foot got caught in the crossover ladder spraining the left ankle.

Employee was operating a low-trac when it quit tramming EE was being pulled outside by a diesel scoop when EE hit something jarring EE hurting lower back, butt, left leg.

Over-exertion in lifting objects

Employee was building cribs, as well as participating in other routine belt line maintenance acts during shift, and stated that EE strained EE's back.

Struck by rolling or sliding object

Rib roll onto back of employee's right leg.
