Struck against stationary object

Maitenance employee was using a piece of thin metal flashing as a straight edge. While moving the flashing, the edge lacerated the back of his left thumb.

Over-exertion (Not Elsewhere Classified)

Employee was packaging 50 lb. bags. He removed a bag from the spout and turned to the left to place the bag on a nearly full pallet. As he turned, he felt a sharp pain in his left leg and he fell to the floor. He received medical attention at the local hospital. He has a left knee strain and is working on restricted duty.

Over-exertion (Not Elsewhere Classified)

Worker was on walkway when a gust of wind knocked him off balance. In grabbing a hand rail, he 'tweaked' his back. Incident occurred on 12/22/2008 but worker did not seek medical treatment until 01/05/2009.

Caught in, under or between a moving and a stationary object

Employee was lifting a roll of stretch wrap when his hand slipped, his right ring finger tip was caught between the stretch wrap and concrete wall fracturing his finger tip.

Over-exertion in pulling or pushing objects

Worker was pushing vacuum hose further onto spout when hand was pulled into vacuum hose.

Struck by falling object

Employee was lifting a calcium metal crown weighing approximately 80 pounds. Employee lost his grip on the crown, dropping it on his right foot.

Employee was shoveling material when he experienced pain in lower back.

Employee was walking up an inclined walkway when he felt a pop in his right calf.

The employee was discharging a furnace when her goggles fogged up, when she went to wipe the goggles off a piece of dust went into her right eye.
