Using a ratchet wrench with an extension bar was being used to loosen hold down bolts on the chipper blades. The employee was pushing the wrench and lost his grip. His hand struck against the metal frame around the chipper, causing a laceration and fracture to the 4th finger of his left hand.
The employee pushed a crown from the pusher cradle to the roller conveyor. As the crown slid onto the roller conveyor, the employees' hand was at the lower portion of the crown headed toward the roller conveyor. One of his fingers were pinched between the crown and one of the rollers. The employee fractured the tip of his right ring finger.
Employee pulling on a chain-fall, when weight shifted on chain-fall the pull chain was pulled from employees hand. The employee tried to hold the chain, but it was pulled from his grasp causing injury to his right bicep muscles.
Temporary staffing employee was using a razor knife and cutting excess tape from a vacuum hose when he accidentally cut his left thumb. He sustained a superficial laceration to the left thumb.
Employee was a opening a valve and felt a strain in his left shoulder. He continued to work without restrictions.On 11/28/12 he requested a medical appointment.During late January he reported that he is receiving physical therapy. He is still working full duty.
