Struck against a moving object

Employee reported discomfort and knot in groin area at end of shift.

Struck against stationary object

Employee working under suspended portion of bucket belt trying to twist broken belt for removal. Employee went to get out from under belt and struck a bucket from the belt with his ribs.

Fall to the walkway or working surface

While walking on snow covered ice from the Quarry Lunchroom the employee slipped and fell backwards injuring his left shoulder and right hip. No medical treatment initially. On 01/16/2014 he was evaluated at local medical facility and released for full duty with a referral to physical therapy (P.T.) Incident became recordable on 01/24/2014 when he began P.T.
