Over-exertion in lifting objects

Employee was helping shovel rock off of a conveyor when he felt a pop in his right shoulder.

Caught in, under or between a moving and a stationary object

Employee took it upon himself to do a task without training for the task. He replaced the chipper chute and caught the 4th and 5th digits of his right hand between chute and chipper while maneuvering chute. Fracturing a bone in each finger.

Employee had entered the cab of a haul truck and was sitting in the seat. A wind gust caught the door as he was closing it and jerked his arm and shoulder.

Over-exertion in lifting objects

Employee was lifting a water bottle onto the water dispenser in the Production lunchroom when he strained his back.

Struck against a moving object

Worker was driving a haul truck when he noticed pain in his shoulder. He had been experiencing this pain for over a year, but it was bothering him enough that he wanted to see a doctor. We have not determined yet if this is work related.
