Caught in, under or between a moving and a stationary object

Right index finger tip pinched between v belt and a sheave during a belt change. The injury was a fracture which needed surgical repair so the employee lost three workdays while getting medical attention. The employee returned to work on 8/12/19.

Over-exertion in wielding or throwing objects

The employee snipped the bands off a pallet located in the bed of a pickup with a wire snips and felt a twinge in their back. At first appointment on 12/5, the provider restricted the employee to desk work until an MRI was completed, but did not indicate it was work related. The neurologist provided a report on 2/5 that said it is work related.

Caught in, under or between running or meshing objects

A Temporary employee was in process of loading an incoming truck, when the mast chain drive became loose by hitting a bump on the roadway. They decided to try and put the chain drive back in place by placing their left hand inside the chain pulley, when the pulley snapped back in place, catching the left finger, causing a fracture.

Struck against a moving object

The Haul Truck Operator driving a 777D unit was traveling with a load to the Quarry Crusher, when the operator veered from roadway, & traveled through an approximate 5'7"" berm. The truck rolled down an approximate 15-20 embankment & landed upright at the next roadway area. The operator allegedly nodded off & lost control of the haul truck.
