Struck against stationary object

Employee was preparing to line up a bolt on the wash screen. The bonnet of the wash screen moved forward which resulted in the employee ""jerking"" their hand away, which resulted in the employee suffering a laceration to their right index finger. The employee received 7 to 8 sutures to close the wound.

Struck against a moving object

Inattention to surroundings by the operator. In addition, task fixation (focusing on the stacker being down) was a contributing factor.
