Employee installing a return roller and was struck in the mouth by a 2.5 pound sledge hammer that glanced off the roller.

Struck by flying object

Employee was preparing to removing a camlock from the rear of the water truck. It was stuck, so they attempted to tap the fitting with a hammer and it had pressure built up and it struck the employee in the mouth.

Caught in, under or between a moving and a stationary object

Employee pinched a hand in between a hinged step and the skid steer bucket that was assisting them.
