The water level in the settling pond increased higher than past years and washed out over the top of the berm releasing approximately three feet of water from the pond, leaving the ground level at 4"" - 6"" deep in the immediate area. No employees working in area. New overflow pipe installed and the dike has been repaired.

Operator had his bed raised washing material out of got back in truck and failed to lower bed before pulling away and caught the power lines and snapped electric pole. We left operator in machine until power could be terminated.

Had a fire start in mill control room that lasted a short time. We installed a new control room. Fed the power from a different transformer bank, and ran new power supply.
Flash burns (electric)
Employee was working on replacing a twenty amp circuit breaker. Employee de-energized panel by turning 100 amp main circuit breaker off. Leads on primary side of main circuit breaker shorted and caused flash.
