Employee was lifting a rock from inside the Gyro Crusher mantel. He hit is left hand on the spider causing his right hand to twist, fracturing a bone in his right hand. His right hand was fractured in an injury sustained at home and not at work previously. His right hand that was fractured was pinned. The twisting motion of the right hand re-fractured the bone.
Employee was shoveling rocks when she experienced back pain
Employee was working on the clean up crew. She was doing work in the laydown yard picking up steel balls from the ground under a platform and putting them into drums. While moving from right to left she twisted her left knee.
Operator was cutting a bag of lime powder open when a cloud of lime powder came into the operator's face. The operator did not have safety googles on & powder got into the operators left eye.
The main air header isolation valve was closed and the pressure relief drain valve was open. When the operator opened the quick disconnect coupling the pressure in the header popped open the header and dust was blown all around and got dust in operator's right eye.
While looking for stock items a warehouse employee stepped across a beam on a storage rack, slipped and went through two separate openings which caused scraping to the legs. Freezing rain had caused a slippery surface on the beam.
Mill Operator slipped on ice outside door entry which caused an ankle fracture.
Employee cleaning up material from a pump box. He had just finished up jackhammering the material and was shoveling out the loose material. The employee felt pain in his lower back. He stated that he was twisting side to side while shoveling and not moving his whole body at the time of injury.
A journeyman and apprentice powerhouse operator were performing routine maintenance on a lube oil purifier. They were carrying a cone stack of approximately 80 pounds up two flights of steps, where is was to be cleaned. The load was placed on the deck at which time the apprentice realized a pain in his side and labored breathing. He was sent off site for further evaluation.
Employee has undergone knee surgery for an injury which was initially reported on April 16, 2007 and classified as a first aid. The employee developed right knee pain while climbing ladder in the mill concentrator. Continued physician evaluation is now requiring probable orthroscopic surgery for final diagnosis. Employee has been at work without restriction.
Employee stepped on a rock and twisted his right ankle while exiting a vehicle.
Employee stepped in a small ground depression and twisted his left foot. Employee continued to work favoring him left foot. This action of favoring his left foot caused employee's right knee to injury months later requiring surgery.
EE was working small confined area removing 2 pumps when he noticed a dull pain in his lower back.
Employee was sitting in the chair of a drill cab operating the drill string control when he had a sharp pain in his lower right back.
Employee was working around the mantle of the gryo crusher removing rocks and gravel. While lifting a large rock out of the dump pocket, the rock slipped smashing his right pinky finger between two rocks.
Employee was rebuilding a pump in the mill concentrator, during the rebuild the employee stated that he was bending and stooping over the pump when he noticed pain from his back area. Employee was seen by our P.A. and was placed on restricted duty.
Employee reported that his new boots had caused a severe pain in his right foot. The boots were new and he was in breaking them in, when he developed a soreness in the foot. Employee was put on restricted duty.
Employee was uncoupling an air hose that was contaminated with rust and dirt when he was blasted with air from hose connection.
Employee turned & reached to pick up a tool when he experienced pain in left hip and was unable to bear weight on leg.
Employee was operating equipment using his left hand on control lever. During shift employee began experiencing left wrist pain from repeditive wrist motion.
Another worker dropped a construction timber which rolled onto the left hand of the injured employee while he was positioning a second timber. Fractured 4th digit of the left hand.
EE was lifting with his arms several times per day for four days in a row. EE noticed some pain in his right are and was sent to the clinic. EE was placed on light duty starting 2-22-07
Employee was closing a pump valve when he pinched his finger from the pump handle.
Employee was removing pump barrel from housing with a piece of plate to protect the pump shaft and a sledge hammer when a glancing blow knocked the plate off striking him in the right shin.
Employee was working with an impact wrench working overhead in an awkward position.
The employee was moving grating onto the forks of a forklift and cut a one inch laceration on the right hand ring finger. The employee was not wearing the proper gloves. The employee received 3 sutures, and was released to full duty.
Employee was removing old piping from the lead feed pumps. The section of pipe he was holding onto was on the other side of the handrail. This pipe needed to be raised so that his partner could finish cutting the hugger clamp off. When he raised the pipe, he heard a pop in his left calf. The pipe is ASB and does not weigh that much.
Employee was removing a cargo strap which was secured to a load to a flatbed. The hook of the cargo strap was caught in the shrink wrapping material. The employee pulled the strap towards him which loosened the strap hook and flew back hitting him in the mouth and chipping tooth.
Employee was climbing up the steps to the Caterpillar Compactor when he lost grip of the handrails and fell back onto the ground. The employee received lower back contusion from the fall.
Driver was tightening lug nuts and the wrench slipped while pulling down on it, striking the left 4-5 fingers on the hub cap of the axle.
Employee was cleaning scale from the top of the tower mill when a chunk of scale fell into the man-basket he was using. The employee was pushed into the man-basket control, which caused contusion to the rib area.
Cutting a plastic curtain with box cutter, it slipped causing a cut to left forearm - stitches required only.
While removing a guard from an air compressor, employee turned and twisted lower back.
Mechanic was grinding on a trailer axle and got a piece of metal in his eye. He was wearing safety glasses but not required faceshield. Minor injury resulted.
Employee wore the wrong size work gloves, which contributed to the development of a skin rash and thumbnail infection.
Employee was assisting with brush removal on the airport broom when he felt a poping noise folled bt a sharp pain in his lower back. A half hour later his back started tighting up and he could not sit down.
EE was operating a fork when he observed a thin strap that was broke on his load. He stopped the fork lift and walked around to pull of the strap and sliped with his hand and cut his thumb requiring stiches.
Employee was in process of removing bolts with hand tool when the socket broke & caused him to twist & fall to the floor. He was orginally treated as a 1st Aid only. He coninued to work until recently when he began experiencing increased right knee pain. He had right knee surgery 5/24/07.
Employee lacerated his hand with a knife. Knife blade was facing hand.
Employee had his annual hearing test done and records show that he has progressive hearing loss.
