Struck against stationary object

Driver was exiting truck and struck head on the door frame causing a strain to the neck

Caught in, under or between a moving and a stationary object

Mechanic injured hand when pinched between handle of jack and truck body while placing jack to perform work. Mechanic continued working without issue for remainder of 4 week rotation. While on R&R mechanic went to doctor and was not able to return site for scheduled rotation on 12/03/2020.

Fall from machine

Mechanic was preparing to dismount the tracks of a CAT 336 excavator. As EE was dismounting the grease line hung on the track. Mechanic attempted to shake the hose loose and kick it free; ultimately losing balance and making contact with the ground below with the right shoulder.

Fall to the walkway or working surface

Wash bay attendant was pulling the fire hose back into the bay. This was being done by pulling and backing up at the same time and tripped over the hose falling and hitting EE's head.

Struck against stationary object

During civil construction activities, operator stepped down from the skid-steer EE was operating, onto an uneven ground surface (a rock) causing EE's ankle to roll inward, causing strain-like pain. Medical evaluation determined a hair-line fracture had occurred in the left ankle.

Bodily reaction, (Not Elsewhere Classified)

Employee twisted right knee while working on electrical instrumentation. Employee reported to PA for evaluation and returned to work without restrictions. Employee continued to work until going out on R/R 12 April.On 30 April employee notified PA that they were diagnosed with partial meniscus tear requiring surgery.

Caught in, under or between a moving and a stationary object

Employee was assisting removing the trunnion liner which broke loose of its interference fit and fell uncontrollably. The employee stumbled back and EE's foot became trapped between flange and floor.

Employee struck EE's left hand with a hammer while tightening the jam nut on the jaw crusher lower stationary side liner bolt.

Struck by flying object

EE was grinding and felt something enter EE's eye. EE continued working. At lunch EE flushed it. After lunch EE's eye was still irritated. Reported incident, went to clinic, PA drilled out an embedded foreign body. No rust ring present.

Struck by flying object

EE pulled down welding hood and felt something in EE's eye. EE flushed the eye and proceeded to finish EE's task. No other issue with the eye. The following day EE's eye was irritated when the sun came out. EE flushed it again. Around lunch time it was still irritated. EE informed the Supervisor and Safety escorted to on site PA. A rust ring developed requiring it to be drilled.

IP woke up with an embedded metallic foreign body in eye. Unknown when it happened. Seen by on site PA, foreign body was removed by drilling of the eye.

Fall onto or against objects

Employee was walking through the Dry Storage Warehouse. Attempting to walk around and over the fork, right boot caught the fork, stumbled and tripped. Employee reached out right had to catch themselves and grabbed a part of the rack that had a steel burr that cut the right 5th finger.

Caught in, under or between a moving and a stationary object

Employee was removing the suction spool on a slurry pump when their left hand was pinched between the control valve frame and suction spool. This resulted in a laceration to the left ring finger.

Contact with hot objects or substances

Employee was hot steaming a frozen HDPE line. Employee reached down through the exhaust steam cloud to grab the steam hose and was burned on the right wrist.
