Caught in, under or between (Not Elsewhere Classified)

Employee was moving a portable ramp that had folding legs underneath. Ramp was positioned against the wall and as the employee started to lay the ramp on the floor a set of the folding legs fell out and caught his left hand index finger in a pinch point. Suffered a severe laceration and broken bone to this finger.

Caught in, under or between a moving and a stationary object

Employee was removing broken concrete from mixer truck drum through a hole cut in the side. As he pushed 1 piece out his left thumb was pinched between the edge of the hole and the piece of concrete, causing a laceration and a broken bone on the tip of the thumb.

Struck against stationary object

Employee was performing maintenance on product Pb slurry filter press chain drive assemblies when employees left hand made contact with a bracket causing a minor puncture to palm.

Over-exertion in lifting objects

Employee was lifting one side of portable generator to completely drain the fuel tank before permorming further maintenance. Employee set generator down and stood up, at that point the employee felt a ""pop"" in his lower back. The Supervisor was notified and the employee was sent to an onsite medical clinic for evaluation.

Over-exertion in pulling or pushing objects

Employee was pulling a lube hose from a hose reel on the service lube truck when hose became difficult to pull. Employee continued to pull on hose more aggressively when he felt discomfort in right shoulder.

Contact with hot objects or substances

Employees changing outboard motors on skiff. Change was complete, decking reinstalled and secured. Tachometer gage for one of outboards reading high. Employee turned key to activate ignition source. Explosion occurred. 3 employees received minor injuries. Boat had damage to decking and motors and was immediately removed from service.

Over-exertion (Not Elsewhere Classified)

Employee was dismounting from the bed of a flatbed delivery truck, placed right foot on the ground, the ground was uneven and as a result the employee rolled right ankle.

Struck by falling object

Employee grabbed the tongue of a trailer to move it off to the side so it would be out of the way, while pushing the the trailer to the side the trailer tongue jack collapsed and fell on the employees right foot. The jack was found to be fully in the locked position. Employee suffered a acute contusion to the top of right foot.

Equipment operator was attempting to put a guard back on a in-cab defrost fan while the fan was still running. His hand slipped and made contact with the fan blades.

Employee cut hand with a pocket knife while preparing hoses to cover the threads on the adjustment bolts for a motor. Hose was not secured while cutting and use of improper cutting technique attributed to injury.

Caught in, under or between a moving and a stationary object

Rolling a truck tire out of the restraining tire cage using one hand. Weight shifted on the tire and wheel, causing the middle finger on the left hand to be pinched between the tire and the tire cage bars.

Contact with hot objects or substances

Employee cutting shim plates with torch. Lost focus and passed right wrist through torch when reaching for shim. Not in compliance with PPE policy, not wearing gloves required for task by policy and rolled down gloves he was wearing exposing wrist and arm. Cause - incorrect PPE and lack of focus

Struck by falling object

Drill shack roof support failed. Roof strut and 3/8"" plywood fell onto Driller's hard hat. Driller was knocked unconscious. Driller recovered later but has continuing hearing loss in left ear. Transported for further evaluation.

Struck against stationary object

Employee was installing new man door and got left hand caught on a 5"" screw and injured 4th digit on left hand.

Employee was on the way to assigned job by 2007 PB transfer pump, next to water colletion tank. While in route he crouched down to go under a low hanging pipe and hit his head on a vic coupling.

There was a missing vent on the A/C vents upon which the employee stuck his hand up into and struck the fan that was rotating. This caused a laceration on the left middle finger of the employee.

Contact with hot objects or substances

Employee was using a torch to cut rod iron. While holding the torch in his right hand and lifting with his left hand the torch spun and burnt the employee on his left wrist between his jacket sleeve and glove cuff.

Absorption of radiations, caustics, toxic and noxious substances

Employee was emptying a supersack of lime into a hopper. Employee was cutting the bottom of the bag when lime enter into right eye. Employee was wearing goggles over glasses, however the goggles were not properly fit and this caused a gap at the temples of the glasses.
