Caught in, under or between a moving and a stationary object

A miner was fatally injured while using a tool to remove a down hole hammer. The drill motor turned unexpectedly, pinning the driller's leg between the tool and the drill mast.

Struck by falling object

Mechanics were removing bolts from transmission bellhousing in equipment yard. At approximately 3:45 pm. Foreman came to rear of dozer to check on I.P's progress and noted that IE had the transmission ready to secure to prevent slippage. The foreman and 3rd mechanic were preparing rigging to secure the transmission when the transmission slipped and came to rest on I.P's left leg.

A Warehouse employee was helping the Load Master, Pilot, & Co-Pilot push a pallet that was stuck. The pallet moved forward slightly then stopped. While the Warehouse employee was waiting to push again, the load master lowered the pallet on the pallet jack and pushed the arm/handle of the pallet jack forward, crushing the Warehouse employee's hand between the arm & the base

Employee had cut out a gussett out of 2"" thick steel. The gussett would not come free when the employee tried to break it loose. The employee used two pry bars to get it free. In the process one of the pry bars slipped which caused the metal to fall and pinch the employees left hand middle finger.

working in a tight area with limited tooling that can fit in the area.

Over-exertion (Not Elsewhere Classified)

While installing delineators on the new B-line ramp, an operator sprained an ankle on the uneven ground by the berm. Operator did not see a rock and stepped on it which sprained the ankle.

Over-exertion in lifting objects

Employee was transferring booster from the powder truck to a five gallon bucket to distribute them to the blast holes they where going to load. When employee picked up the bucket felt something go wrong with their lower back.

Employee was working on a sump pump that was secured to table with a come-along. Two eye bolts used on the suction case liner for pick points to be removed and handled with crane. When the liner came off the pump teetered pinching employees finger between pump and suction liner.

Over-exertion in lifting objects

Employee strained right shoulder while lifting material for warehouse order.

Over-exertion in pulling or pushing objects

Employee strained back while assisting team and the load master reposition by pushing a pulling a snow plow in a cargo plane.

Caught in, under or between a moving and a stationary object

While moving PQ core tube, the drill helper lost footing on a clear work area. The tube fell from EE's grasp and EE re-grabbed the tube while it fell. EE's hand came in contact with a pipe 12"" above deck level (door frame pipe). This contact with EE's hand between the tube and pipe resulted in a fracture to EE's Right Ring Finger.

Struck by flying object

IP had just finished work for the day. IP stopped at the smoke area before heading in to camp. IP felt discomfort in left eye and it started watering. IP was seen by the onsite PA. PA diagnosed a rust ring, drilled the eye, prescribed antibiotic drops and released IP to work. IP had been grinding on pipe during shift.

Struck by flying object

Worker was handling panel at grade on work benches. Was attempting to knock snow and ice off a panel by repeatedly picking up and dropping it on the work bench. The wind caught the panel and worker tried to dump it off the bench. It twisted and one corner popped up and contacted cheek, resulting in a minor laceration requiring sutures.

Employee was snipping metal banding off of a palletized item at the warehouse. When the metal banding was snipped the banding sprang toward the employee striking the arm area causing a laceration that required two sutures from the onsite medical staff.

Struck against stationary object

Employee was gathering a new set of gear to put on the bolts above EE. When EE got up, EE smacked a bolt with their mouth and broke a tooth.

IP and co-worker were moving a 12"" reinforced rubber hose that had a wire barb sticking out the end. As the IP lifted the end, the wire barb punctured EE's leg. IP received sutures from the on site PA.

Contact with cold

Driller was operating the Diedrich D-120 Drill during shift while wearing adequate PPE combined with shelter and heat in winter conditions. Post-Shift 11/21 the driller noticed signs of possible frostbite. EE continued to work the next day and did not report EE's symptoms until post-shift on 11/22. EE was treated for frostbite and sent off-site for further evaluation.
