Over-exertion in lifting objects

On October 21, 2018, Port Operator was shoveling lead/zinc concentrate spillage underneath the P28 conveyor belt. On October 23, 2018 the employee notified the Port Medic of a sore back. The employee was sent to the Mine for evaluation by the PA. The employee was sent off-site for further medical evaluation.

Struck by flying object

Mechanic was installing track pin with the 5P2380 track tool unit when the ram adapter fractured into 3 pieces. The mechanic was contacted on the right side of the face resulting in lacerations and fracture to the jaw.

Struck by falling object

A contract supervisor was inspecting the work area. The supervisor who was wearing fall protection stepped onto a section of floor grating which had been left unsecured, it fell 25' striking a Mill operator on head. The injured Mill operator walked away from the incident scene after preliminary medical assessment and flown offsite for further evaluation.

Struck against stationary object

Mill Operator injured left thumb while operating a hand crank to manually open a roll-up door.

The drill crew was tripping out drill rods and laying them in the rod tray located on the side of the drill rig. The employee was using a wrench to raise the rod to loosen the rod hoisting bail. Once the hoisting bail was removed the employee pulled out the wrench causing the rod to shift and come in contact with EE's right hand.

Struck against stationary object

While constructing scaffolding, IP struck right knee against stationary pipe rack, resulting in bruising and swelling.

Over-exertion in lifting objects

Operator was shoveling media from ball mill trough when arm started aching. The following day employee was climbing the feeder ladder when arm started to hurt.

Struck against a moving object

Operator lost control of a fully loaded Caterpillar 777 F Model haul truck while attempting to make a left hand turn into a dump area. When applying the brakes ee lost traction and the equipment rolled over onto it's right side.

Contact with hot objects or substances

Employee sustained a second degree burn on right index finger knuckle while working on instrumentation tubing next to an exhaust manifold.

Struck against stationary object

Employee visited medic complaining of pain in thumb due to a metal sliver, no foreign object was located. Employee returned the following day complaining of additional pain, still no foreign object found. Medic issued antibiotics to employee.

Fall onto or against objects

Employee was crossing over a 10"" and 26"" HDPE pipe. Employee stepped onto the 10"" pipe to get over the 26"" pipe slipped and right knee landed on 26"" HDPE pipe.

Rubbed or abraded

Employee was pulling electric cable while kneeling under the construction camp kitchen when they developed bursitis and an infection in the left knee. EE was treated at onsite medical clinic and released to full work duty.
