Fall from machine

Employee was stepping out of the equipment when EE's left foot slipped from the step causing EE to fall to the ground. Employee felt pain in the shoulder area.

Employee was attempting to enter the D7 dozer in the pit. As the operator reached for the top hand rail, slipped and fell backwards, landing on the ground. Employee suffered small contusions on back and right arm. Doctors put employee on restrictive duty.

Employee walking to get into service truck, stepped into a low uneven ground area, felt pain in R knee. Employee was able to grab hold of service truck door to keep from falling.

Employee pulled on a rubber soap hose and it struck him in his left eye. Causing pain and redness, he was seen by doctor and released to regular duty, with a follow-up appointment on the 1-28-08.
