Employee was stepping out of the equipment when EE's left foot slipped from the step causing EE to fall to the ground. Employee felt pain in the shoulder area.

Employee was attempting to enter the D7 dozer in the pit. As the operator reached for the top hand rail, slipped and fell backwards, landing on the ground. Employee suffered small contusions on back and right arm. Doctors put employee on restrictive duty.

Employee walking to get into service truck, stepped into a low uneven ground area, felt pain in R knee. Employee was able to grab hold of service truck door to keep from falling.

Employee pulled on a rubber soap hose and it struck him in his left eye. Causing pain and redness, he was seen by doctor and released to regular duty, with a follow-up appointment on the 1-28-08.

Employee was working by crusher & a large rock fell and hurt his right ankle.

EE was working with a sledgehammer in maintance area and smashed his finger.

EE was using a cutting torch. EE was wearing safety glasses and a piece of hot metal flew into his eye.

A rock fell off shaker screen and hit his left foot.

HE WAS GREASING THE ROLL CRUSHER & HIGH PRESSURE FROM THE PNEUMATIC GREASE GUN INJECTED GREASE INTO HIS FINGER.

HE SLIPPED WHILE EXITING TRUCK AND FELL TO THE GROUND.

EMPLOYEE WAS DRIVING A PIN IN WITH A HAMMER THE PIN CHIPPED OFF AND FLEW INTO THE EMPLOYEE'S (L)EYE. DAMAGE OR INPAIRMENT IS UNKNOWN AT THIS TIME.

EE WAS HOLDING A WRENCH WHEN HE WAS HIT ON THE FINGER BY A HAMMER HELD BY ANOTHER EE, THE PLANTMANAGER AT THE TIME
