Accident type, without injuries

A roof fall occurred in No. 2 Entry of TG-203. The fall occurred at the No. 16 x-cut. The intersection fell approximately 20 feet wide by 20 feet long and was approximately 10 feet high.

Accident type, without injuries

A roof fall occurred between the shield tips and the longwall face. This fall blocked travel off the face into the tailgate. The tailgate entry remained open. The fall measured approx 5 ft H by 5 ft L by 6 feet W. The fall occurred at a faulted area on the face.

The roof in the tailgate fell and blocked the safe travel off of the longwall face. The fall was approx 4 to 6 feet high, 12 feet long and 16 feet wide.

A roof fall and subsequent tailgate blockage occurred at approx 4:35 pm. The fall obstructed travel off the tailgate end of the longwall face.

A tailgate blockage occurred as the face reached survey station 2507. The fall was approx. 10 ft high, 20 ft wide and 25 ft long. Initial support included 7 ft torque tension bolts, welded wire mesh supplemented by cable trusses and roc props.

Fall from machine

Operator of LDW 208 loader slipped from the access ladder & fell 6 feet causing him to fracture his left leg.

Struck against a moving object

EE WAS DRIVING THE SHUTTLE CAR AND EVERY TIME HE HIT A BUMP, HE HAD PAIN IN HIS BACK & NECK. THE PAIN WORSENED AS HE CONTINUED TO HIT THE BUMPS.

Fall to the walkway or working surface

Emp slipped on oily surface & twisted Left knee while repairing hyd leak. NOTE: This became LTA on Mar 7th due to employee having surgery on injured knee

Fall from machine

EMPLOYEE FELL APPROX 5' OFF CANOPY OF BOLTER AND STRUCK HIS HEAD ON THE HOSE OF THE TRACK. THE FALL RESULTED INTO A LACERATION TO HIS RIGHT EAR AND NECK PAIN

Struck by falling object

A rib 4'x3'x6"" came off and pinned ee against the frame of main gate. ee was standing sideway and the rib hit him in the left rid area. Ee pshed the rid away standing sideway and the rib hit him in the left rid area. EE pushed the rid away from him and reported the incident and continued to try and work but couldn't due to pain.

Struck by falling object

A pressurized rock prop was dropped and either the rock prop end or pressurized air struck employee on right upper thigh causing contusion.

Struck by flying object

EE (ROOF BOLTER OPERATOR) WAS STRUCK IN THE HEAD BY A TRUSS ANCHOR SHOE DURING INSTALLATION OF THE TRUSS. THE TRUSS LEG BROKE DURING INSTALLATION AND THE SHOE WAS THROWN OFF STRIKING THE OPERATOR ABOVE THE LEFT EYE, RESULTING IN A LACERATION APPROX 3"" LONG

Fall to the walkway or working surface

EE slipped and fell backward while walking across the travel pad, causing him to twist his knee. Did not seek medical treatment, until 2-15-05 and was released full by the doctor. 2-17-05 due to increase of pain the doctor released him to light duty.

EE missed his step and fell, while repairing light cord on shields, causing his right foot and left to jammed between the shields and struck his right shoulder against the shield.

There was a lrg coal spill on belt 17 headroller. Coal was being moved from coal spill to an openhole in the top of the surge bin that had not been used for this purpose in the past. The injured emp did not know that his hole was open and the emp came around the front of 17 belt chute and stepped into the open hole.

While the crew were exiting quickly from the work area, employee fell and re-aggravated her old injury to her right ankle.

EMPLOYEE PINCHED & LACERATED HIS FINGER WHILE CHANGING THE BELT ROLLERS. THE DR. DIAGNOSED HIM WITH A FRACTURE & DISLOCATION OF HIS LEFT SECOND FINGER.

While employee was pulling on a knotted cable he stepped on an uneven surface causing him to fall on his right side and strain his right knee.

WHILE EMPLOYEE WAS PULLING ON A KNOTTED CABLE HE STEPPED ON AN UNEVEN SURFACE CAUSING HIM TO FALL ON HIS RIGHT SIDE AND STRAIN HIS RIGHT KNEE.

WHILE EMPLOYEE WAS SHOVELING COAL HE FELT A POP AND SHARP PAIN IN HIS FOREARM.

EMPLOYEE TWISTED HER ANKLE WHEN SHE STEPPED ON A PARTIALLY BURIED ROCK, WHILE EXITING FROM THE UNIT (GEHL LOADER).

Struck by falling object

A SLAB OF COAL CAME OFF THE RIB STRIKING EE ON THE BACK AND RIGHT LEG, RESULTING IN A CONTUSION. 7/15/05: EE WAS RELEASED FULL DUTY FROM CLINIC. 7/27/05: EE WAS RELEASED TO SEDENTARY WORK FROM CLINIC DUE TO CONTINUOUS PAIN AND CONTUSION.

WHEN EMPLOYEE WAS LOADING TRUSS SHOES ONTO THE BOLTER TRAY, HIS FINGER CAUGHT BETWEEN THE SHOES, RESULTING INTO A LACERATION TO HIS RIGHT INDEX FINGER

Struck by falling object

Employee placed his hand between drill box and a drill steel that was hung up in the roof. The steel fell unexpectedly, fracturing employee's middle finger.

When the cable reel was turned on, employee caught her left leg between the cable and rib, resulting in a contusion

Employee was checking the inlet filter with the vacuum pump still running, as he was closing the lid for the fileter, the suction slammed the lid shut from about 4"" of being closed, the lid caught employee's little finger, causing him to dislocate his finger.

While employee was removing a drill steel, the drill steel came loose smashing the tip of employee's right ring finger between the drill steel and drill box.

The employee, a tire repair person, was assisting another employee replace a tire when a pry bar he was using to hold a lug nut slipped and pinched his left middle finger. Injury required 5 stitches.

Employee fell on a sharp piece of coal and received a deep cut, requiring stitches.

Emp states that he was removing a 1/4"" plastic tie wrap from a high PSI hydraulic hose using a 3"" razor knife when he cut his left thumb. Emp received 7 stitches.

Inhalation of radiations, caustics, toxic and noxious substances

While working at a section ee received acute chemical exposure, resulted to loss of consciousness.

Struck by falling object

While removing cover from joyminer, the cover fell sideway pinching and lacerating ee's left finger and hand.

Absorption of radiations, caustics, toxic and noxious substances

Employee reported what seemed like an insect bite to foreman on 05/25/05. He thinks he may have gotten bit while attending annual refresher training away from the mine site on 05/23/05. He was taken to Clinic on 05/25/05, two days after alleged incident date. He was given oral and injected antibiotics, and place on restricted duty.
