Over-exertion (Not Elsewhere Classified)

Was at pond 11 checking water pump fuel level. While walking slid off the berm into the mud. Right foot stuck in the mud about 5"" deep and I twisted ankle, pain also felt in the lower knee area.

Fall onto or against objects

while stepping from push arm on left side of dozer to track lost footing and fell on right side back/rib on the track grousers

Struck against a moving object

Reported neck was hurting from loader hitting or rock dropped in bed of truck. Went to Urgent Care for treatment on 10/6, did not notify employer until next day. Employer requested 2nd opinion from Reliance Medical Center. Released to full duty 10/7 with follow up appointment on 10/14, follow up referred chiropractic treatment. Received chiropractic treatment on 10/20.
